替格瑞洛与氯吡格雷辅助常规药物治疗冠心病心肌缺血患者的效果对比

Comparison of the Effects of Ticagrelor and Clopidogrel on Conventional Drugs in the Treatment of Patients with Coronary Heart Disease and Myocardial Ischemia

目的:对比替格瑞洛与氯吡格雷治疗冠心病心肌缺血患者的效果。方法:选取2022年1月—2023年6月本院收治的120例冠心病心肌缺血患者,随机分成对照组和研究组,各60例。两组病例均接受常规治疗,在此基础上,对照组给予氯吡格雷,研究组给予替格瑞洛。比较两组血小板抑制率、血液流变学指标、心绞痛发作情况、心肌损伤指标、心功能指标。结果:治疗后,研究组ADP、AA以及COL均高于对照组(P<0.05);研究组全血高切黏度、全血低切黏度和红细胞压积均低于对照组(P<0.05);研究组心绞痛发作次数少于对照,心绞痛发作时的单次持续时间短于对照组(P<0.05);研究组肌钙蛋白Ⅰ、肌酸激酶同工酶和乳酸脱氢酶均低于对照组(P<0.05);研究组NT-proBNP和左心射血分数均优于对照组(P<0.05)。结论:冠心病心肌缺血患者治疗时,应用替格瑞洛对血小板聚集的拮抗效果优于氯吡格雷,可有效改善血液流变学,减少心绞痛发作和心肌损伤,有利于改善心功能。

Objective:To compare the effect of ticagrelor and clopidogrel in the treatment of myocardial ischemia in patients with coronary heart disease.Methods:A total of 120 patients with myocardial ischemia of coronary heart disease admitted to our hospital from January 2022 to June 2023 were randomly divided into control group and study group,with 60 patients in each group.Both groups received routine treatment.On this basis,the control group was given clopidogrel,and the study group was given ticagrelor.The platelet inhibition rate,hemorheology index,angina pectoris attack,myocardial injury index and cardiac function index were compared between the two groups.Results:After treatment,ADP,AA and COL in the study group were higher than those in the control group(P<0.05).The whole blood high shear viscosity,whole blood low shear viscosity and hematocrit in the study group were lower than those in the control group(P<0.05).The number of angina pectoris attacks in the study group was less than that in the control group,and the single duration of angina pectoris attacks was shorter than that in the control group(P<0.05).The levels of troponin I,creatine kinase isoenzyme and lactate dehydrogenase in the study group were lower than those in the control group(P<0.05).NT-proBNP and left ventricular ejection fraction in the study group were lower than those in the control group(P<0.05).Conclusion:In the treatment of patients with myocardial ischemia of coronary heart disease,the antagonistic effect of ticagrelor on platelet aggregation is better than that of clopidogrel,which can effectively improve hemorheology,reduce angina pectoris and myocardial injury,and improve cardiac function.
