国医大师黄瑾明以“气血均衡”理论辨治食管癌术后经验

摘要 文章系统总结国医大师黄瑾明基于“气血均衡”理论辨治食管癌术后经验。黄教授认为气血失衡为食管癌发生发展之本,“虚、毒、瘀、郁”互结为食管癌术后的病机特点,病性为虚实夹杂,病位在食管,涉及脾、胃、肝、肾,且与脾、胃关系密切。治疗上以均衡气血,调整阴阳为原则,提出解毒、补虚、祛瘀、调气四大治法。解毒推崇“以毒攻毒”,善用岭南“毒药”,补虚以脾胃为先,注重使用血肉有情之品,祛瘀强调以补血为要,针药并用,同时善用壮医脐环针调节全身气机,作为食管癌治疗的重要一环;最后附验案1则,以期为本病治疗提供新思路。 To summarize systematically the experience of TCM master HUANG Jinming in treating postoperative esophageal cancer based on the‘balance of qi and blood’theory.Professor HUANG believes that imbalance of qi and blood is the root cause of esophageal cancer,and‘deficiency,toxicity,stasis and depression’combine with each other to characterize the pathogenesis of postoperative esophageal cancer,and the pathology is a mixture of deficiencies and realities,and the disease is located in the esophagus,involving the spleen,stomach,liver,and kidneys,but it is closely related to the spleen and stomach.The treatment is based on balancing qi and blood,adjusting yin and yang,and proposing the four major treatments of detoxification,replenishing deficiency,regulating qi,and resolving blood stasis.Detoxification advocates‘fighting poison with poison’,and makes use of Lingnan‘poison’;supplementation of deficiency takes spleen and stomach as the first priority,and focuses on the use of blood and flesh sentient products;elimination of blood stasis emphasizes blood tonicity as the main point,and the use of needles and medicines,and at the same time,makes use of the Qihuanzhen of Zhuang medicine to regulate the body’s qi,which is an important link in the treatment of esophageal cancer.In the end,a case is attached,in order to provide new ideas for the treatment of this disease.
