断指再植成活率的影响因素分析

Analysis of influencing factors of survival rate of replantation of severed fingers

目的:分析断指再植成活率的影响因素并探究应对措施。方法:选取2019年3月至2023年10月我院收治的112例(136指)末节断指再植患者为研究对象,根据术后断指存活情况分为存活组(n=78,95指)和未存活组(n=34,41指),比较两组临床资料,采用logistic回归分析断指再植成活率的影响因素。结果:断指保存方式为冷藏、伤后就诊时间<6h、术后疼痛<3分、术后断指颜色良好、术后断指指腹弹性良好和术后断指毛细血管反应良好是断指再植成活率的保护因素(P<0.05);年龄>60岁、断指损伤类型为完全离断、术后发生感染、术后发生血管危象和术后断指皮肤温度低于33℃是断指再植成活率的危险因素(P<0.05)。结论:断指再植成活率的影响因素较多,临床可针对不同因素采取相应干预措施,以提高患者术后断指成活率。

Objective:To analyze the influencing factors of the survival rate of replantation of severed fingers and explore countermeasures.Methods:A total of 112 patients(136 fingers)undergoing replantation of severed distal fingers admited to our hospital from March 2019 to October 2023 were selected as the research objects.According to the survival status of severed fingers after surgery,they were divided into a survival group(n=78,95 fingers)and a non-survival group(n=34,41 fingers).The clinical data were compared between the two groups,and logistic regression was used to analyze the influencing factors of the survival rate of replantation of severed fingers.Results:Refrigerated preservation of severed fingers,treatment time after injury<6 hours,postoperative pain<3 points,good postoperative finger color,good postoperative fingertip elasticity and good postoperative capllary reaction of severed fingers were protective factors for the survival rate of replantation of severed fingers(P<0.05).Age>60 years old,complete amputation of severed finger injury type,postoperative infection,postoperative vascular crisis and postoperative severed finger skin temperature lower than 33 C were risk factors for the survival rate of replantation of severed fingers(P<0.05).Conclusion:There are many influencing factors on the survival rate of replantation of severed fingers.Clinically,corresponding intervention measures can be taken according to different factors to improve the survival rate of severed fingers after surgery.
