Sigma⁃1受体与阿尔茨海默病认知功能及盐酸多奈哌齐临床疗效的相关性

Correlation of Sigma-1 Receptor with Cognitive Function and Clinical Efficacy of Donepezil Hydrochloride in Alzheimer's Disease

摘要 目的探究阿尔茨海默病(AD)患者血清Sigma-1受体表达水平,分析其潜在临床价值。方法选取2022年3月至2023年12月收治的80例中轻度AD患者,随机分为认知训练组(认知功能训练)和多奈哌齐组(认知功能训练联合盐酸多奈哌齐),每组40例,同期选取40例健康体检者作为健康对照组。检测健康对照组体检当天血清Sigma-1受体水平、蒙特利尔认知评估量表(MoCA)和简易智力状态检查量表(MMSE)评分。治疗前及治疗8周后,检测认知训练组和多奈哌齐组血清Sigma-1受体水平以及MoCA、MMSE、临床痴呆评定量表(CDR)和日常生活活动能力评定量表(ADL)评分。采用Pearson相关性分析治疗后血清Sigma-1受体水平与MoCA、MMSE、ADL和CDR评分的相关性。结果与健康对照组比较,认知训练组和多奈哌齐组血清Sigma-1受体、MoCA和MMSE评分均降低(P<0.05),认知训练组与多奈哌齐组比较差异无统计学意义(P>0.05)。治疗后,多奈哌齐组血清Sigma-1受体、MoCA、MMSE和ADL评分均较治疗前和认知训练组升高,而CDR评分较治疗前和认知训练组降低(P<0.05)。治疗后,AD患者血清Sigma-1受体与MoCA、MMSE和ADL评分呈正相关性,而与CDR评分呈负相关性(P<0.01)。结论AD患者血清Sigma-1受体水平与认知功能相关,盐酸多奈哌齐治疗能够提高患者血清Sigma-1受体表达量,改善患者的记忆认知功能。 Objective To investigate the expression level of serum Sigma-1 receptor in patients with Alzheimer's disease(AD)and to analyze its potential clinical value.Methods Eighty patients with mild-to-moderate AD admitted from March 2022 to December 2023 were randomly divided into cognitive training group(cognitive function training)and Donepezil group(cognitive function training combined with Donepezil Hydrochloride),with 40 patients in each group,and 40 healthy physical examinees were selected as healthy control group during the same period.Serum Sigma-1 receptor levels,Montreal Cognitive Assessment Scale(MoCA)and Mini-mental State Examination Scale(MMSE)scores of all health controls were measured on the day of physical examination.Before and at 8 weeks after treatment,serum Sigma-1 receptor levels and scores of MoCA,MMSE,Clinical Dementia Rating Scale(CDR)and Activities of Daily Living Rating Scale(ADL)in the cognitive training group and Donepezil group were detected.Pearson correlation analysis was used to analyze the correlation between serum Sigma-1 receptor level and MoCA,MMSE,ADL and CDR scores after treatment.Results Serum Sigma-1 receptor,MoCA and MMSE scores in cognitive training group and Donepezil group were decreased compared with healthy control group(P<0.05),but there was no significant difference between cognitive training group and Donepezil group(P>0.05).After treatment,serum Sigma-1 receptor,MoCA,MMSE and ADL scores in Donepezil group were higher than those before treatment and those in cognitive training group,while CDR scores were lower than those before treatment and those in cognitive training group(P<0.05).After treatment,serum Sigma-1 receptor in AD patients was positively correlated with MoCA,MMSE and ADL scores,but negatively correlated with CDR scores(P<0.01).Conclusion The level of serum Sigma-1 receptor is related to cognitive function in AD patients.Treatment with Donepezil Hydrochloride can increase the expression of serum Sigma-1 receptor and improve the memory and cognitive function of patients.
