香砂八珍汤对老年食管癌放疗患者的疗效观察

Curative Effect of Xiangsha Bazhen Decoction(香砂八珍汤)on Elderly Patients With Esophageal Cancer Treated By Radiotherapy

目的探究香砂八珍汤对老年食管癌放疗患者的治疗疗效。方法选取2021年1月—2022年12月老年食管癌放疗患者120例,采用随机数表法分为观察组(62例)和对照组(58例)。对照组接受常规化疗,观察组在常规化疗基础上,给予香砂八珍汤治疗。对比两组治疗疗效以及不良反应发生情况。治疗前后,检测患者中医证候评分、营养指标[白蛋白(albumin,ALB)、转铁蛋白(transferritin,TRF)以及血红蛋白(hemoglobin,Hb)]、免疫指标(CD_(4)^(+)、CD_(8)^(+))和转录因子(NF-kB p65、NF-kB p50)蛋白含量变化。结果观察组、对照组疾病控制率(disease control rate,DCR)分别为66.13%、50.00%,差异无统计学意义(P>0.05)。治疗后,观察组吞咽哽噎、四肢困乏、食欲不振和舌苔薄腻证候评分均低于对照组(P<0.05),证候脉弦紧、胸膈痞满得分无差异(P>0.05)。治疗后,观察组血清ALB、Hb和TRF浓度高于对照组,血浆CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)表达水平高于对照组,血浆CD_(8)^(+)低于对照组,差异均具有统计学意义(P<0.05)。治疗后,观察组癌组织NF-kB p50、NF-kB p65蛋白含量低于对照组,差异具有统计学意义(P<0.05)。观察组恶心呕吐、白细胞下降不良反应发生率低于对照组(P<0.05)。结论香砂八珍汤可有效改善老年食管癌放疗患者的营养状况和机体免疫功能,降低癌组织中NF-κB蛋白含量和不良反应发生风险,对肿瘤控制的影响并无显著。

Objective To explore the therapeutic effect of Xiangsha Bazhen Decoction(香砂八珍汤)on elderly patients with esophageal cancer treated by radiotherapy.Methods Between January 2021 and December 2022,a total of 120 elderly patients with radiotherapy for esophageal cancer were randomly selected and divided into observation group(62 cases)and control group(58 cases)by random number table method.The control group received conventional chemotherapy,and the observation group was given Xiangsha Bazhen Decoction on the basis of conventional chemotherapy.The therapeutic effect and the occurrence of adverse reactions were compared between the two groups.Before and after treatment,TCM syndrome scores,nutritional indexes[albumin(ALB),transferritin(TRF)and hemoglobin(Hb)],immune indexes(CD_(4)^(+),CD_(8)^(+))and protein contents of nuclear factor kappa-B(NF-kB)transcription factor(NF-kB p65,NF-kB p50)were detected.Results The disease control rate(DCR)of observation group and control group was 66.13%and 50.00%,respectively,with no statistical significance(P>0.05).After treatment,the syndrome scores of swallowing and choking,limb drowsiness,loss of appetite and thin and greasy tongue coating in the observation group were lower than those in the control group(P<0.05),but the scores of tight pulse and full chest and diaphragm fullness had no difference(P>0.05).After treatment,the serum concentrations of ALB,Hb and TRF in the observation group were higher than those in the control group.The plasma expression levels of CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+) were higher than those in the control group,and plasma CD_(8)^(+) was lower than that in the control group.The differences were statistically significant(P<0.05).After treatment,the protein contents of NF-kB p50 and NF-kB p65 in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence of nausea,vomiting and leukopenia inthe observation group was lower than that in the control group(P<0.05).Conclusion Xiangsha Bazhen Decoction can effectively improve the nutritional status and immune function of elderly patients with radiotherapy for esophageal cancer,reduce the content of NF-κB protein in cancer tissues and the risk of adverse reactions,but there is no significant effect on tumor control.
