清热消瘀方熏洗对混合痔术后患者疼痛程度、创面愈合及肛门功能的影响

Effect of Qingre Xiaoyu Prescription(清热消瘀方)Fumigation and Washing on Pain Degree,Wound Healing and Anal Function of Patients after Mixed Hemorrhoids Operation

目的研究清热消瘀方熏洗对混合痔术后患者疼痛程度、创面愈合及肛门功能的影响。方法2020年1月—2022年6月东台市人民医院收治的混合痔术后患者90例,分为对照组与试验组,各45例,方法为随机数字表法。对照组予以甘霖洗剂治疗,试验组予以清热消瘀方熏洗治疗,两组均治疗2周。比较两组治疗2周后疗效,治疗前、治疗2周后症状积分、凝血功能,不同时间点肛门功能,治疗期间并发症发生情况。结果治疗2周后,与对照组的总有效率(80.00%,36/45)比较,试验组总有效率更高[95.56%(43/45),P<0.05]。治疗2周后两组疼痛、水肿、出血症状评分降低,且试验组更低(P<0.05)。治疗前、治疗1周后、治疗2周后,两组肛管静息压呈降低趋势,且试验组更低(P<0.05);肛管最大收缩压呈升高趋势升高,且试验组更高(P<0.05)。治疗前、治疗2周后两组活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、凝血酶时间(thrombin time,TT)以及凝血酶原时间(or prothrombin time,PT)比较,差异无统计学意义(P>0.05)。治疗过程中,与对照组并发症发生率17.78%(8/45)比较,试验组并发症发生率更低[4.44%(2/45),P<0.05]。结论清热消瘀方熏洗治疗混合痔术后患者疗效确切,可改善患者疼痛、水肿、出血等临床症状,提高肛门功能,促进创面愈合,可降低术后并发症的发生。

Objective To study the effect of Qingre Xiaoyu Prescription(清热消瘀方)fumigation and washing on pain degree,wound healing and anal function of patients after mixed hemorrhoids surgery.Methods From January 2020 to June 2022,90 patients with mixed hemorrhoids underwent surgery in Dongtai People′,s Hospital were divided into the control group(45 cases)and the experimental group(45 cases),the method was random number table method.The control group was treated with Ganlin lotion,and the experimental group was treated with Qingre Xiaoyu Prescription fumigation and washing.Both groups were treated for 2 weeks.The efficacy of two groups after 2 weeks of treatment,scores of symptoms,coagulation function,anal function at different time points before and after 2 weeks of treatment and complications during treatment were compared.Results After 2 weeks of treatment,the total effective rate of the experimental group was higher[80.00%(36/45)vs 95.56%(43/45),P<0.05].After 2 weeks of treatment,the scores of pain,edema and bleeding symptom of the two groups decreased,and the scores of the experimental group were lower(P<0.05).1 and 2 weeks after treatment,the anal canal maximum systolic pressure showed an increasing trend in the two groups,and was higher in the experimental group(P<0.05).The anal canal resting pressure showed an decreasing trend in the two groups,and was lower in the experimental group(P<0.05).There was no significant difference in activated partial thromboplastin time(APTT),thrombin time(TT)or prothrombin time(PT)between the two groups before treatment and after 2 weeks of treatment(P>0.05).During the treatment,the complication rate of the experimental group was lower(4.44%,2/45)compared with that(17.78%,8/45)of the control group(P<0.05).Conclusion Qingre Xiaoyu Prescription fumigation and washing was effective in the treatment of patients with mixed hemorrhoids after operation.It could improve the clinical symptoms such as pain,edema and bleeding and improve the anal function,and reduce the occurrence of postoperative complications.
