高压氧联合海藻酸钠口腔用水凝胶对慢性牙周炎患者炎症指标及牙周微生态的影响

Effects of hyperbaric oxygen combined with sodium alginate oral hydrogel on inflammatory markers and periodontal microbiota in patients with chronic periodontitis

摘要 目的探讨高压氧(HBO)联合海藻酸钠口腔用水凝胶对慢性牙周炎(CP)患者炎症指标及牙周微生态的影响。方法选取2020年3月至2023年3月胜利油田中心医院口腔科收治的CP患者90例作为研究对象,采用随机数字表法分为观察组(n=45)和对照组(n=45)。对照组采用海藻酸钠口腔用水凝胶治疗,研究组采用海藻酸钠口腔用水凝胶联合HBO治疗。比较2组患者的临床疗效,治疗前后牙周微生态指标、Barthel咀嚼功能及疼痛视觉模拟评分(VAS)的变化;记录治疗期间不良反应的发生情况。采用酶联免疫吸附法测定血清白细胞介素(IL)-1β、IL-6、肿瘤坏死因子-α(TNF-α)及龈沟液Shh蛋白、Dickkopf-1、HC-gp39水平。结果治疗4个疗程后,研究组患者治疗总有效率(91.11%)明显高于对照组(73.33%),差异有统计学意义(χ^(2)=4.865,P=0.027)。研究组患者治疗后牙周微生态指标较对照组明显改善(P<0.01)。治疗后研究组患者VAS明显低于对照组,Barthel咀嚼功能评分明显高于对照组(P<0.01)。与治疗前比较,2组患者治疗后血清IL-6、IL-1β、TNF-α及龈沟液Shh蛋白、Dickkopf-1、HC-gp39水平均明显降低,且治疗后研究组明显低于对照组(P<0.01)。在治疗过程中,研究组患者不良反应总发生率(4.44%)与对照组(13.33%)比较差异无统计学意义(χ^(2)=2.195,P=0.138)。结论HBO联合海藻酸钠口腔用水凝胶可以明显降低CP患者的炎症反应,改善牙周微生态,提高临床治疗效果。 Objective:To study the effects of hyperbaric oxygen(HBO)combined with sodium alginate oral hydrogel on inflammatory markers and periodontal microbiota in patients with chronic periodontitis(CP).Methods:A total of 90 patients with CP admitted to Department of Stomatology of Shengli Oilfield Central Hospital from March 2020 to March 2023 were selected as the study subjects.They were randomly divided into study group(n=45)and control group(n=45)using the random number table method.The control group was treated with sodium alginate oral hydrogel,while the study group was treated with sodium alginate oral hydrogel combined with HBO.The changes in clinical efficacy,periodontal microbiota indicators,Barthel chewing function,and visual analog scale(VAS)of pain between two groups before and after treatment were compared,and the occurrence of adverse reactions during the treatment period was recorded.Enzyme linked immunosorbent assay was used to measure the levels of serum interleukin(IL)-1β,IL-6,tumor necrosis factor-α(TNF-α),and Shh protein,Dickkopf-1,and HC-gp39 in gingival crevicular fluid.Results:After 4 courses of treatment,the total effective rate of the study group(91.11%)was significantly higher than that of the control group(73.33%),and the difference was statistically significant(χ^(2)=4.865,P=0.027).The study group showed significant improvement in periodontal microbiota indicators compared with the control group(P<0.01).The VAS of the study group was significantly lower than that of the control group,and the Barthel chewing function score was significantly higher than that of the control group(P<0.01).Compared with before treatment,the levels of serum IL-6,IL-1β,TNF-α,gingival crevicular fluid Shh protein,Dickkopf-1,and HC-gp39 in both groups were significantly reduced after treatment,of which the levels of study group were significantly lower than those of the control group after treatment(P<0.01).During the treatment,the difference of the total incidence of adverse reactions between the study group(4.4%)and the control group(13.33%)was not statistically significant(χ^(2)=2.195,P=0.138).Conclusion:Hyperbaric oxygen combined with sodium alginate oral hydrogel can obviously reduce the inflammatory reaction of patients with chronic periodontitis and improve the periodontal microbiota,thus improving the clinical treatment effect.
