某舰3批次执行护航任务期间官兵运动系统损伤调查分析

Investigation and analysis of the incidence of musculoskeletal injuries in officers and sailors of a warship during three escort missions

摘要 目的探讨某补给舰3批次执行护航任务期间官兵常见运动系统损伤发生特点与规律,并探讨其相关危险因素,为提高护航医疗保障效能提供参考。方法回顾性分析某补给舰3批次护航期间医疗就诊记录,统计舰员运动系统损伤的发生率、分布特点和发生规律;同时选取最近批次护航编队舰员(n=224)进行运动系统损伤相关因素问卷调查,根据是否发生运动系统损伤,将舰员分为损伤组(n=47)和无损伤组(n=177),进行单因素和多因素回归分析,确定与运动系统损伤相关的独立危险因素。结果某补给舰3批次执行护航任务期间官兵运动系统损伤呈逐年下降趋势,主要损伤部位集中在肩、腰、膝、足/踝。最近批次224名男性舰员中47人例存在运动系统损伤,单因素分析结果显示,2组舰员运动前进行热身/运动后进行拉伸、是否佩戴护具、单次训练时间和参与的训练类型可能是运动系统损伤的重要影响因素(P<0.05或P<0.01);多因素Logistic回归分析结果显示,运动前进行热身/运动后进行拉伸、佩戴护具、单次训练时间和参与的训练类型是运动系统损伤发生相关的独立危险因素(P<0.05或P<0.01)。结论运动系统损伤是护航官兵的常见疾病;探索诱发护航官兵运动系统损伤的常见危险因素,为预防运动损伤的发生提供参考。 Objective To investigate the features and pattern of occurrence of musculoskeletal injuries in officers and sailors of a warship during three escort missions and to explore the relevant risk factors,so as to provide references for improving the efficiency of medical support during escort missions.Methods Medical records of three escort missions were analyzed retrospectively and the incidence,distribution features,and occurrence patterns of musculoskeletal injuries were statistically analyzed.Meanwhile,the crew members of the latest escort mission(n=224)were selected to conduct a questionnaire survey on the factors related to musculoskeletal injuries.According to the occurrence of musculoskeletal injuries,the participants were divided into two groups:injury group(n=47)and non-injury group(n=177).Then univariate and multivariate regression analyses were performed to determine the independent risk factors related to the incidence of musculoskeletal injuries.Results There was a downward trend year by year of the incidence of musculoskeletal injuries in officers and sailors of a warship during three escort missions.Shoulder,low back,knee,and foot/ankle were the most frequently injured parts.Among the 224 male subjects,47 had musculoskeletal injuries.Univariate analysis showed that warm up before exercise/stretching after exercise,wearing protective gear,training time,and training type were possibly the important factors(P<0.05 or P<0.01).Multivariate logistic regression analysis showed warm up before exercise/stretching after exercise,wearing protective gear,training time,and training type were the independent risk factors related to musculoskeletal injuries(P<0.05 or P<0.01).Conclusion Musculoskeletal injury is common among officers and sailors during escort missions.Efforts should be made to explore the common risk factors of musculoskeletal injuries occurrence among officers and sailors during escort missions,thus providing references for the prevention of musculoskeletal injury.
