温针灸透刺治疗卒中软瘫疗效观察

Efficacy observation of penetrating needling by warming needle moxibustion on cerebral stroke-induced flaccid paralysis

目的:观察温针灸透刺法与现代康复手段相结合对脑卒中软瘫期患者患侧肢体运动障碍的治疗效果以及对核磁弥散张量成像的影响。方法:将60例脑卒中软瘫期肢体障碍患者随机分为两组各30例。两组均给予常规基础治疗及头针治疗,对照组予常规针刺联合神经肌肉关节促进法治疗;治疗组予温针灸透刺联合神经肌肉关节促进法治疗。比较两组治疗前后FA值、MBI评分、FMA评分的变化。结果:治疗后两组患者FA值均较治疗前增长,MBI评分、FMA评分均有所提高(P﹤0.05),且治疗组各评分均明显优于对照组(P﹤0.05)。结论:温针灸透刺联合康复治疗可改善卒中后软瘫期患者的肌力、运动功能障碍、日常生活活动能力,且弥散张量成像证实该疗法对皮质脊髓束完整性的恢复有一定的促进作用。

Objective:To observe the clinical efficacy of penetrating needling by warming needle moxibustion combined with modern rehabilitation on lateral limbs dyskinesia of patients in the stage of cerebral stroke-induced flaccid paralysis and the effect on diffusion tensor imaging of nuclear magnetic resonance(NMR).Methods:60 cases of patients with limbs dyskinesia in the stage of cerebral stroke-induced flaccid paralysis were randomly divided into two groups,30 cases in each group.At the first,both groups were treated by routine basic therapy and scalp acupuncture;then,the control group was treated by routine acupuncture and neuromuscular joint facilitation techniques,while the treatment group was treated by penetrating needling by warming needle moxibustion and neuromuscular joint facilitation techniques.The changes of FA value,MBI score and FMA score were compared between two groups before and after treatment.Results:In both groups after treatment,FA value of patients was increased compared with pre-treatment,MBI score and FMA score were also increased to a certain degree(P﹤0.05),and each score of treatment group was significantly better than that of control group(P﹤0.05).Conclusion:Penetrating needling by warming needle moxibustion combined with modern rehabilitation therapy can improve the muscle strength,dyskinesia and activities of daily living of patients with cerebral stroke-induced flaccid paralysis.And DTI confirms that this therapy can promote the recovery of corticospinal tract integrity to some extent.
