可吸收螺钉内固定术在掌指骨斜形骨折患者中的应用效果

Effect of Internal Fixation Surgery with Absorbable Screws in Patients with Metacarpophalangeal Oblique Fractures

目的:探讨可吸收螺钉内固定术在掌指骨斜形骨折患者中的应用价值。方法:回顾性分析2020年1月—2024年2月兰州市安宁区万里医院收治的84例掌指骨斜形骨折患者资料,根据手术方法将其分为研究组和对照组,各42例。所有患者均予以常规切开复位,对照组采用微型钢板进行内固定,研究组采用可吸收螺钉进行内固定。比较两组手术指标、术后恢复情况、手功能改善情况、血清指标变化、并发症发生情况。结果:研究组手术时间、骨折愈合时间均短于对照组,恢复工作时间长于对照组,差异均有统计学意义(P<0.05)。研究组手功能恢复的优良率为97.62%,高于对照组的80.95%,差异有统计学意义(P<0.05)。术后3 d,两组白细胞介素-6、肿瘤坏死因子-α水平均较术前升高,差异均有统计学意义(P<0.05),但两组组间比较,差异均无统计学意义(P>0.05)。两组术中出血量、切口长度比较,差异均无统计学意义(P>0.05)。研究组并发症发生率为7.14%,低于对照组的14.29%,但两组比较,差异无统计学意义(P>0.05)。结论:对掌指骨斜形骨折患者采用可吸收螺钉内固定术,可以缩短手术耗时、加快术后恢复、改善早期手功能,但术后恢复工作的时间稍长于微型钢板,临床可根据患者的实际情况选择恰当的手术方法。

Objective:To investigate the application value of absorbable screw internal fixation surgery in patients with metacarpophalangeal oblique fracture.Methods:Data of 84 patients with metacarpal oblique fractures admitted to Anning District Wanli Hospital of Lanzhou City from January 2020 to February 2024 were retrospectively analyzed,and were divided into a study group and a control group according to surgical methods,with 42 cases in each group.All patients were treated with conventional open reduction,microplate was used for internal fixation in the control group,absorbable screws were used for internal fixation in the study group.The surgical indexes,postoperative recovery,hand function improvement,serum indexes and the occurrence of complications were compared between the two groups.Results:The operation time and fracture healing time of the study group were shorter than those of the control group,and the recovery time of the study group was longer than that of the control group,the differences were statistically significant(P<0.05).The excellent and good rate of hand function recovery in the study group was 97.62%,which was higher than 80.95%in the control group,the difference was tatistically significant(P<0.05).At 3 days after operation,the levels of interleukin-6 and tumor necrosis factor-αwere increased in both groups,the differences were statistically significant(P<0.05),but there were no significant differences between the two groups(P>0.05).There were no significant differences in intraoperative blood loss and incision length between the two groups(P>0.05).The complication rate in the study group was 7.14%,which was lower than 14.29%in the control group,but there was no significant difference between the two groups(P>0.05).Conclusion:The use of absorbable screws for internal fixation of patients with metacarpophalangeal oblique fractures can shorten the operation time,speed up postoperative recovery and improve early hand function,but the postoperative recovery time is longer,and the appropriate surgical method can be selected according to the actual situation of patients.
