摘要
目的:探讨无创呼吸机辅助治疗对慢性阻塞性肺疾病(COPD)合并呼吸衰竭(RF)患者的效果。方法:选取2019年1月—2023年12月泰安市第一人民医院收治的60例COPD合并RF患者,按随机数字表法将其分为对照组和研究组,各30例。对照组采用常规治疗,研究组在对照组基础上联合无创呼吸机辅助治疗。比较两组血气指标、炎症指标、症状缓解时间及肺功能指标。结果:治疗后,研究组动脉二氧化碳分压低于对照组,血氧分压及血氧饱和度均高于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组白细胞介素-6、C反应蛋白、肿瘤坏死因子-α均低于对照组,差异均有统计学意义(P<0.05)。研究组咳嗽、咳痰、胸闷及呼吸困难的缓解时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组用力肺活量、最大呼气峰值流速、第1秒用力呼气容积均高于对照组,差异均有统计学意义(P<0.05)。结论:无创呼吸机辅助治疗可加快纠正COPD合并RF患者的血气指标异常,缓解疾病症状,促进炎症消散,改善患者肺功能。
Objective:To investigate the effect of non-invasive ventilator assisted treatment on patients with chronic obstructive pulmonary disease(COPD)complicated with respiratory failure(RF).Methods:A total of 60 patients with COPD combined with RF admitted to Tai’an First People’s Hospital from January 2019 to December 2023 were selected.They were divided into a control group and a study group according to random number table method,with 30 cases in each group.The control group received conventional treatment,the study group was supplemented with non-invasive ventilator assisted treatment on the basis of the control group.Blood gas index,inflammation index,symptom remission time and lung function index were compared between the two groups.Results:After treatment,the arterial carbon dioxide fraction of the study group was lower than that of the control group,the blood oxygen partial pressure and blood oxygen saturation were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the interleukin-6,C-reactive protein and tumor necrosis factor-αin the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The remission time of cough,sputum,chest tightness and dyspnea in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05).After treatment,forced vital capacity,maximum peak expiratory flow rate and forced expiratory volume at the first second in the study group were higher than those in the control group,with statistical significance(P<0.05).Conclusion:Non-invasive ventilator assisted treatment can accelerate the correction of blood gas abnormalities in COPD patients with RF,alleviate disease symptoms,promote inflammation dissipation,improve lung function.
