双低剂量的下肢CTA在糖尿病下肢动脉硬化闭塞症患者中的应用效果研究

Application effect of double low dose lower extremity CTA in diabetic patients with lower extremity arteriosclerosis obliterans

目的研究双低剂量的下肢CT血管造影(CTA)在糖尿病下肢动脉硬化闭塞症(DLASO)患者中的应用效果。方法选取2021年1月-2023年12月天津市滨海新区中医医院放射科就诊的80例DLASO患者资料进行回顾性分析,按照掷硬币的方案分为A组(40例,100kV、270mgI/mL、1.5mL/kg)和B组(40例,120kV、350mgI/mL、1.5mL/kg),比较两组图像质量主观、客观评分及辐射剂量。结果两组图像质量主观评分比较,无统计学差异(P>0.05)。两组CT值、背景噪声比较,无统计学差异(P>0.05);两组背景信号、信噪比、对比噪声比比较,A组低于B组(P<0.05)。与B组比较,A组CT剂量指数、有效辐射剂量、人均对比剂碘摄入剂量更低(P<0.05)。结论采用双低剂量的下肢CTA检测DLASO时,可有效保证图像质量,同时降低辐射剂量和减少碘摄入剂量,值得推荐。

Objective To study the effect of double low-dose lower extremity CT angiography(CTA)in patients with diabetic lower extremity arteriosclerotic obliteration(DLASO).Methods The data of 80 DLASO patients in the Radiology department of Binhai New Area Hospital of Traditional Chinese Medicine of Tianjin from January 2021 to December 2023 were selected for retrospective analysis.According to the coin toss scheme,they were divided into group A(40 cases,100kV,270mgI/mL,1.5mL/kg)and group B(40 cases,120kV,350mgI/mL,1.5mL/kg),and the subjective and objective score indexes of image quality and radiation dose of the two groups were compared.Results There was no significant difference in the subjective score of image quality between the two groups(P>0.05).There were no significant differences in CT value and background noise between the two groups(P>0.05).The background signal,signal-tonoise ratio and contrast-to-noise ratio of group A were lower than those of group B(P<0.05).Compared with group B,CT dose index,effective radiation dose and per capita iodine intake of contrast agent in group A were lower(P<0.05).Conclusion The use of double low-dose lower extremity CTA to detect DLASO can effectively ensure the image quality,while reducing the radiation dose and iodine intake dose,which is worthy of recommendation.
