不良生活习惯对不同性别高血压患者左心房重构的影响

Effect of Body Mass Index on Left Atrial Remodeling in Hypertensive Patients

目的探讨不良生活习惯对不同性别高血压患者左心房重构的影响。方法将1241例高血压患者分为3组,正常体重组[身体质量指数(BMI)<24 kg/m^(2),n=422],超重组(24 kg/m^(2)≤BMI<28 kg/m^(2),n=633),肥胖组(BMI≥28 kg/m^(2),n=186)。记录患者的各项临床数据以及超声心动图指标数值,采用Pearson相关性分析及多元线性回归分析处理上述数据。结果正常体重组LAD/身高(LADI)值为(20.16±5.00)mm/m,超重组LADI值为(20.99±5.37)mm/m,肥胖组LADI值为(22.46±5.79)mm/m,3组差异有统计学意义(P<0.05),且女性高血压患者LADI显著高于男性,差异有统计学意义(P<0.05);BMI与LADI的相关性(r=0.237,P<0.05)较左心室重量指数(LVMI)高(r=0.173,P<0.05)。多元线性回归分析显示,BMI、年龄、性别与LADI独立相关(P<0.05)且BMI是影响高血压患者LADI的显著因素(男性β=0.229,P<0.001;女性β=0.159,P<0.001),随着BMI的增长,各组LADI随之增加。结论高血压患者的BMI值越高其左心房重构越明显,且男性患者较女性患者显著。

Objective Exploring the impact of unhealthy lifestyle habits on left atrial remodeling in hypertensive patients of different genders.Methods The 1241 hypertensive patients were divided into three groups based on their body mass index(BMI),Normal weight group(BMI<24 kg/m^(2),n=422),overweight group(24 kg/m^(2)≤BMI<28 kg/m^(2),n=633),and obese group(BMI≥28 kg/m^(2),n=186).The clinical data and echocardiographic index values of the subjects were recorded.Pearson correlation analysis and multiple linear regression analysis were used to process the data.Results The LADI value was found to be(20.16±5.00)mm/m in the normal weight group(20.99±5.37)mm/m in the overweight group,and(22.46±5.79)mm/m in the obese group.There was a significant difference in LADI among the three groups(P<0.05).Additionally,LADI was significantly higher in female hypertensive patients than in male hypertensive patients(P<0.05).The correlation between BMI and LADI(r=0.237,P<0.05)was higher than that between left ventricular mass index(LVMI)and LADI(r=0.173,P<0.05).Multiple linear regression analysis revealed that BMI,age,and gender were independently associated with LADI(P<0.05).Among these factors,BMI had the strongest impact on LADI in hypertensive patients(β=0.229,P<0.001 for men;β=0.159,P<0.001 for women).As BMI increased,LADI increased in all groups.Conclusion Left atrial remodeling was more pronounced in hypertensive patients with higher BMI.Additionally,it was more pronounced in male patients than in female patients.
