经颅超声检查黑质强回声结合血流动力学对帕金森病的临床诊断价值

Clinical Diagnostic Value of Transcranial Ultrasonography of Substantia Nigra Combined with Hemodynamics in Parkinson's Disease

摘要 目的探讨经颅超声(TCS)结合血流动力学对帕金森病(PD)的诊断价值。方法选取我院神经内科收治的68例PD患者(PD组)以及同期80例健康体检者(NC组),采用经颅超声联合经颅彩色多普勒超声检查,比较两组中脑黑质回声增强面积以及血流动力学特点的差异。结果PD组的单侧、双侧中脑黑质强回声面积以及黑质阳性率均高于NC组,差异无统计学意义(P>0.05)。经颅彩色多普勒测定的大脑中动脉(MCA)、基底动脉(BA)的舒张期血流速度(Vd)值小于NC组,大脑后动脉(PCA)和椎动脉(VA)的收缩期血流速度(Vs)值也小于NC组,而MCA、BA、VA的搏动指数(PI)、阻力指数(RI)均高于NC组。结论经颅超声可以对中脑黑质以及颅脑血管的血流动力学进行全面评估,进而为患者的早期诊断提供参考。 Objective To evaluate the value of transcranial parenchymal ultrasound combined with hemodynamics in the diagnosis of Parkinson's disease.Methods A total of 68 patients with Parkinson's disease(PD group)and 80 healthy subjects(NC group)admitted to the Department of Neurology of our hospital were selected.Transcranial ultrasound combined with transcranial color Doppler ultrasound were used to examine the echo-enhanced area and hemodynamic characteristics of the substantia nigra between the two groups.Results The echo area of substantia nigra and the positive rate of substantia nigra in PD group were higher than those in control group.The Vd values of MCA and BA measured by transcranial color Doppler were lower than those of NC group,and the Vs values of PCA and VA were also lower than those of NC group,while the PI and RI values of MCA,BA and VA were higher than those of NC group.Conclusion Transcranial ultrasound can comprehensively evaluate the hemodynamics of the substantia nigra and craniocerebral vessels,so as to provide some basis for the early diagnosis of patients.
