大黄芒硝汤联合低分子肝素对高甘油三酯血症性急性胰腺炎患者C反应蛋白、甘油三酯及凝血功能的影响

Effects of Dahuang Mangxiao Decoction Combined with Low Molecular Heparin on C-reactive Protein,Triglyceride and Coagulation Function in Patients with Hypertriglyceridemia Acute Pancreatitis

目的探讨大黄芒硝汤联合低分子肝素治疗高甘油三酯血症性急性胰腺炎(HTG-AP)患者的临床疗效及对实验室指标的影响。方法选取2023年1月至2024年4月在景德镇市第二人民医院治疗的82例HTG-AP患者为研究对象,使用随机数字表法将患者分为两组,其中试验组42例,对照组40例。对照组给予小剂量低分子肝素钙,试验组在对照组基础上联合大黄芒硝汤,对两组疗效、C反应蛋白(CRP)、甘油三酯(TG)及凝血功能进行综合评价。结果试验组的治疗效果显著优于对照组(P<0.05)。治疗后,试验组的腹痛缓解时间、肠道功能恢复时间和平均住院日均短于对照组(P<0.05);两组CRP和TG水平均明显降低,且试验组低于对照组(P<0.05);两组凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)均延长,且试验组高于对照组(P<0.05)。结论大黄芒硝汤联合低分子肝素治疗可以减轻HTG-AP患者的临床症状,降低CRP和TG水平,改善凝血功能,从而促进疾病康复,控制病情进展,并改善临床预后。

Objective To explore the clinical efficacy of dahuang mangxiao decoction combined with low molecular heparin in the treatment of patients with hypertriglyceridemia acute pancreatitis(HTG-AP)and the effect on laboratory indexes.Methods Eighty-two patients with HTG-AP treated from January 2023 to April 2024 in The Second People's Hospital of Jingdezhen were selected,the patients were divided into two groups by using the random number table method,of which 42 cases were in the experimental group and 40 cases were in the control group.The control group was given low-dose low molecular heparin calcium,the experimental group was given dahuang mangxiao decoction on the basis of the treatment of the control group,the therapeutic efficacy,C-reactive protein(CRP),triglyceride(TG),and coagulation function of the patients in the two groups were comprehensively evaluated.Results The total effective rate of treatment of the experimental group was higher than that of the control group(P<0.05).After treatment,the experimental group were shorter than the control group in terms of the time of abdominal pain relief,the time of intestinal function recovery,and the average hospitalisation days(P<0.05);the levels of CRP and TG in both groups were significantly reduced,and the experimental group were lower than those of the control group(P<0.05);PT and APTT in both groups were prolonged,and the experimental group were higher than the control group(P<0.05).Conclusion Dahuang mangxiao decoction combined with low molecular heparin can alleviate clinical symptoms,reduce CRP and TG levels,and improve coagulation function in patients with HTG-AP,thereby promoting disease recovery,controlling disease progression,and improving clinical prognosis.
