二氧化锆全冠与纯钛烤瓷全冠对龈沟液中TNF-α和IL-1β浓度水平及牙周组织健康的影响

摘要 目的探讨二氧化锆全冠与纯钛烤瓷全冠对龈沟液中肿瘤坏死因子-α(TNF-α)和细胞介素-1β(IL-1β)浓度水平及牙周组织健康的影响。方法选取进行上颌前牙修复的患者共89例,按照烤瓷冠材料进行分组,观察组(二氧化锆全冠)44例,对照组(纯钛烤瓷全冠)45例,观察两组牙周组织[龈沟液(GCF)量、牙龈沟深度探诊(GCD)、菌斑指数(PLI)、牙龈出血指数(SBI)]、炎症因子[龈沟液中IL-1β及TNF-α]水平、咀嚼能力[咬合力、咀嚼效率、牙齿动度值(PTV)]以及不良反应。结果两组牙体预备前、修复后1个月及3个月的GCF、GCD、PLI、SBI比较差异无统计学意义(P>0.05)。牙体预备前,两组TNF-α、IL-1β水平比较差异无统计学意义(P>0.05);修复后1个月及3个月,两组TNF-α、IL-1β水平均升高,但观察组升高幅度较对照组小(P<0.05)。两组牙体预备前的咬合力、咀嚼效率、PTV评分对比,结果差异无统计学意义(P>0.05);修复后3个月,观察组咬合力、咀嚼效率较对照组高,观察组PTV评分较对照组低(P<0.05)。观察组不良反应发生率为4.55%,较对照组的20.00%低(P<0.05)。结论与纯钛烤瓷全冠相比,二氧化锆全冠可有效减少牙周炎症反应,提高咀嚼能力,且安全性良好。 Objective To investigate the effects of zirconia full crown and pure titanium porcelain full crown on tumor necrosis factor-α(TNF-α)and interleukin-1β(IL-1β)concentrations in gingival crevicular fluid and periodontal tissue health.Methods A total of 89 patients undergoing maxillary anterior tooth restoration in our department were selected.They were divided into groups according to porcelain crown materials,including 44 cases in the observation group(zirconium dioxide full crown)and 45 cases in the control group(pure titanium porcelain full crown).Periodontal tissue[gingival crevicular fluid(GCF)volume,gingival crevicular depth examination(GCD),plaque index(PLI),gingival bleeding index(SBI)],inflammatory factors[levels of IL-1βand TNF-αin gingival crevicular fluid],masticatory ability[bite force,masticatory efficiency,tooth mobility value(PTV)]and adverse reactions were observed in the two groups.Results There were no significant differences in GCF,GCD,PLI and SBI between the two groups before tooth preparation,1 month and 3 months after tooth repair(P>0.05).Before tooth preparation,there were no significant differences in the levels of TNF-αand IL-1βbetween the two groups(P>0.05).At 1 month and 3 months after repair,the levels of TNF-αand IL-1βwere increased in both groups,but the increases in the observation group were smaller than those in the control group(P<0.05).There were no significant differences in bite force,masticatory efficiency and PTV score between the two groups before tooth preparation(P>0.05).At 3 months after repair,the biting force and masticatory efficiency of the observation group were higher than those of the control group,and the PTV score of the observation group was lower than that of the control group(P<0.05).The incidence of adverse reactions in observation group was 4.55%,which was lower than that in control group(20.00%)(P<0.05).Conclusion Compared with pure titanium porcelain full crown,zirconia full crown can effectively reduce periodontal inflammation,improve chewing ability,and has good safety.
