基于瘀毒交阻理论应用五谷麒麟膏对肛瘘术后大鼠模型创面组织促愈及核因子κB p65表达的影响

Effect of Wugu Qilin Ointment on wound healing and expression of NF-κB p65 in a rat model after anal fistula operation based on the stasis-toxin intersection theory

目的观察五谷麒麟膏对肛瘘术后大鼠模型创面组织促愈及核因子κB(NF-κB)p65表达的影响。方法将60只雄性SD大鼠随机分为五谷麒麟膏组、京万红组、模型组和空白组,每组15只;模型组、五谷麒麟膏组、京万红组全麻麻醉后在背部做创面模型,造模成功后模型组给予凡士林纱条换药,五谷麒麟膏组、京万红组给予相应药膏换药,每日1次。给药后观察各组创面愈合情况;肉芽组织中白细胞介素6(IL-6)、IL-8、肿瘤坏死因子α(TNF-α)水平及肉芽组织中NF-κB p65蛋白表达情况。结果给药7、11、15 d五谷麒麟膏组创面愈合率均高于京万红组同期(P<0.05);五谷麒麟膏组给药3、7、15 d肉芽组织中IL-6、IL-8、TNF-α水平均低于京万红组同期(P<0.05);Western blot检测结果显示,五谷麒麟膏组NF-κB p65蛋白表达均低于京万红组、模型组。结论五谷麒麟膏促进创面愈合的机制可能与抑制NF-κB的表达,减少大鼠肉芽组织中IL-6、IL-8、TNF-α等炎症因子的表达水平,从而减轻机体炎症反应有关。

Objective To explore the influence of Wugu Qilin Ointment on promoting wound healing and expression level of nuclear factor kappa B(NF-κB)p65 in rats after anal fistula operation.Methods A total of 60 male Sprague-Dawley(SD)rats were randomly divided into Wugu Qilin Ointment group,Jingwanhong group,model group and blank group,with 15 rats in each group.Rats in the model group,Wugu Qilin Ointment group and Jingwanhong group were subjected to general anesthesia and wound incision on the back.After the successful modeling,vaseline gauze,Wugu Qilin Ointment and Jingwanhong were given correspondingly once a day.The wound healing of each group was observed.Relative levels of interleukin 6(IL-6),IL-8,and tumor necrosis factor alpha(TNF-α),and the protein expression of NF-κB p65 in granulation tissue were detected.Results The healing rate at 7,11 and 15 days after modeling was significantly higher in the Wugu Qilin Ointment group than that of Jingwanhong group(P<0.05).Relative levels of IL-6,IL-8 and TNF-αin granulation tissue at 3,7 and 15 days were significantly lower in the Wugu Qilin Ointment group than those of Jingwanhong group(P<0.05).Western blot showed a significantly lower protein level of NF-κB p65 in the Wugu Qilin Ointment group than that of Jingwanhong group and model group(P<0.05).Conclusion Wugu Qilin Ointment promotes wound healing after anal fistula operation by downregulating NF-κB,and reducing the expression levels of inflammatory factors like IL-6,IL-8 and TNF-αin rat granulation tissue,thereby reducing the inflammatory response.
