耳穴贴压联合中药外敷对肺癌患者疼痛程度及疼痛介质的影响

Effect of auricular pressure pill intervention combined with herbal external application on pain level and pain mediators levels in lung cancer patients

目的观察耳穴贴压联合中药外敷干预对肺癌患者疼痛及疼痛介质的影响。方法将120例肺癌患者按照随机数字表法分为2组,所有患者均给予规范化癌症三阶梯止痛,在此基础上,对照组60例行常规干预,观察组60例予耳穴贴压联合中药外敷干预。干预后,比较2组干预前后疼痛视觉模拟评分(VAS)与疼痛介质[5-羟色胺(5-HT)、内皮素-1(ET-1)、前列腺素E2(PGE2)、P物质(SP)]水平变化情况,同时以Kolcaba舒适状况量表(GCQ)评估患者舒适度改善情况。结果2组干预后疼痛VAS均较本组干预前降低(P<0.05),观察组干预后低于对照组(P<0.05)。2组干预后5-HT、ET-1、PGE2、SP水平均较本组干预前降低(P<0.05),观察组干预后均低于对照组(P<0.05)。2组干预后GCQ评分均较本组干预前升高(P<0.05),观察组干预后高于对照组(P<0.05)。结论耳穴贴压联合中药外敷有助于进一步改善肺癌患者疼痛介质表达,减轻癌痛程度,继而提升患者舒适度。

Objective To explore the effect of auricular pressure pills combined with herbal external application intervention on pain level and pain mediators levels in lung cancer patients.Methods 120 patients with lung cancer were divided into the control group and the observation group by a random number table.All patients were given standardized cancer three-step pain relief,the control group was additionally given routine nursing intervention,while the observation group was additionally given auricular pressure pill combined with herbal external application intervention.The changes in visual analogue scale(VAS)score and pain mediator levels including 5-hydroxytryptamine(5-HT),endothelin-1(ET-1),prostaglandin E 2(PGE 2),substance P(SP)of the two groups were compared,and the improvement of patients'comfort was assessed by the Kolcaba General Comfort Questionnaire(GCQ)scale at the same time.Results After intervention,the both group had significantly pain VAS scores,reduced levels of 5-HT,ET-1,PGE 2,SP,elevated GCQ scores(all P<0.05),the observation group was superior to the control group for the above indexes(all P<0.05).Conclusion Auricular pressure pill intervention combined with herbal external application in lung cancer patients can help to further improve the expression of pain mediators,reduce the degree of cancer pain,and subsequently enhance patient comfort.
