血清TSGF、β-Klotho、卵泡抑素与分化型甲状腺癌分化程度、淋巴结转移情况的相关性

Correlation Between Serum TSGF,β-Klotho,Follicle Suppressin and the Degree of Differentiation and Lymph Node Metastasis in Differentiated Thyroid Cancer

目的探讨血清肿瘤特异性生长因子(TSGF)、β-Klotho、卵泡抑素(FS)与分化型甲状腺癌患者分化程度、淋巴结转移情况的相关性。方法选取2022年2月至2023年2月新郑华信民生医院收治的198例分化型甲状腺癌患者作为病例组,另选同期收治的206例甲状腺结节患者作为对照组。比较两组血清TSGF、β-Klotho、FS水平,对比不同临床病理特征分化型甲状腺癌患者血清TSGF、β-Klotho、FS水平,用Pearson相关性分析分化型甲状腺癌患者血清TSGF、β-Klotho、FS水平之间的相关性,绘受试者工作特征(ROC)曲线,分析血清TSGF、β-Klotho、FS对分化型甲状腺癌的诊断价值。结果病例组血清TSGF、FS水平高于对照组(P<0.05),血清β-Klotho水平低于对照组(P<0.05)。甲状腺滤泡状癌、低分化及有淋巴结转移的分化型甲状腺癌患者血清TSGF、FS水平高于甲状腺乳头状癌、中/高分化及无淋巴结转移的分化型甲状腺癌(P<0.05);血清β-Klotho水平低于甲状腺乳头状癌、中/高分化及无淋巴结转移的分化型甲状腺癌(P<0.05)。Pearson相关性分析结果显示,分化型甲状腺癌患者血清TSGF水平与血清β-Klotho水平呈负相关(r=-0.657,P<0.05),与血清FS水平呈正相关(r=0.594,P<0.05);血清β-Klotho水平与血清FS水平呈负相关(r=-0.712,P<0.05)。血清TSGF、β-Klotho、FS联合检测诊断分化型甲状腺癌的曲线下面积(AUC)为0.941,高于血清TSGF、β-Klotho、FS单独检测(0.827、0.843、0.870,P<0.05)。结论血清TSGF、FS水平在分化型甲状腺癌患者中呈高表达,而血清β-Klotho水平呈低表达,三者可参与患者分化程度、淋巴结转移过程,且三者联合检测对分化型甲状腺癌的诊断价值更高。

Objective To investigate the correlation between serum tumor specific growth factor(TSGF),β-Klotho,follicle suppressin(FS)and the degree of differentiation and lymph node metastasis in patients with differentiated thyroid cancer.Methods A total of 198 patients with differentiated thyroid cancer admitted to Xinzheng Huaxin People’s Hospital from February 2022 to February 2023 were selected as the case group,and 206 patients with thyroid nodules admitted during the same time were selected as the control group.The serum levels of TSGF,β-Klotho and FS were compared between the two groups,and the serum levels of TSGF,β-Klotho and FS in patients with differentiated thyroid carcinoma and different clinicopathological features were compared.Pearson correlation analysis was used to analyze the correlation between serum levels of TSGF,β-Klotho and FS in patients with differentiated thyroid carcinoma.Receiver operating characteristic(ROC)curve was drawn to analyze the diagnostic value of serum TSGF,β-Klotho and FS in differentiated thyroid carcinoma.Results The serum levels of TSGF and FS in the case group were higher than those in the control group(P<0.05).The serum level ofβ-Klotho was lower in the control group(P<0.05).The serum levels of TSGF and FS in patients with follicular thyroid carcinoma,poorly differentiated thyroid carcinoma and differentiated thyroid carcinoma with lymph node metastasis were higher than those in patients with papillary thyroid carcinoma,moderately/well differentiated thyroid carcinoma and differentiated thyroid carcinoma without lymph node metastasis(P<0.05).The serum level ofβ-Klotho was lower than that of the differentiated thyroid carcinoma with papillary thyroid carcinoma,moderately/well differentiated thyroid carcinoma and no lymph node metastasis(P<0.05).Pearson correlation analysis showed that there was a negative correlation between serum level of TSGF and serum level ofβ-Klotho in patients with differentiated thyroid cancer(r=-0.657,P<0.05).There was a positive correlation with serum level of FS(r=0.594,P<0.05).Serum level ofβ-Klotho was negatively correlated with the serum level of FS(r=-0.712,P<0.05).The area under the curve(AUC)value of the combined detection of TSGF,β-Klotho and FS in the diagnosis of differentiated thyroid carcinoma was 0.941,which was higher than that of the single detection of TSGF,β-Klotho and FS(0.827,0.843,0.870,P<0.05).Conclusion The serum levels of TSGF and FS were highly expressed in patients with differentiated thyroid cancer,while the serum level ofβ-Klotho was low.The three could participate in the differentiation degree and lymph node metastasis process of patients,and the combined detection of the three had higher diagnostic value for differentiated thyroid cancer.
