摘要
目的对比经鼻高流量湿化氧疗(HFNC)、持续正压通气(CPAP)治疗支气管扩张(BE)伴肺部感染的效果。方法选取青岛大学附属泰安市中心医院2022年1月—2023年9月收治的86例BE伴肺部感染患者为研究对象,按随机数字表法将其分为对照组及观察组,每组43例。对照组行CPAP,观察组行HFNC,均持续治疗2周。对比两组临床疗效、动脉血气分析及并发症发生情况。结果观察组的治疗总有效率为95.35%,高于对照组的79.07%,差异有统计学意义(P<0.05)。观察组的并发症发生率为4.65%,低于对照组的18.60%,差异有统计学意义(P<0.05)。治疗后,观察组的氧分压为(86.95±6.13)mmHg,高于对照组的(79.53±5.20)mmHg,二氧化碳分压为(37.59±2.67)mmHg,低于对照组的(45.24±3.20)mmHg,组间差异有统计学意义(P<0.05)。结论HFNC能够更好地改善BE伴肺部感染患者的动脉血气分析指标水平,且并发症少。
Objective To compare the efficacy of High-Flow Nasal Cannula Oxygen Therapy(HFNC)and continuous positive airway pressure ventilation(CPAP)in the treatment of bronchiectasis(BE)with pulmonary infection.Methods A total of 86 patients with BE with pulmonary infection admitted to Tai'an Central Hospital Affiliated to Qingdao University from January 2022 to September 2023 were selected as the study objects and divided into a control group and an observation group according to random number table method,with 43 cases in each group.The control group received CPAP and the observation group received HFNC.Both groups were treated for 2 weeks.The clinical efficacy,arterial blood gas analysis indexes and complications were compared between the two groups.Results The total effective rate of the observation group was 95.35%,which was higher than 79.07%of the control group,the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 4.65%,which was lower than 18.60%in the control group,the difference was statistically significant(P<0.05).After treatment,the partial pressure of oxygen in the observation group was(86.95±6.13)mmHg,which was higher than(79.53±5.20)mmHg in the control group,and the partial pressure of carbon dioxide was(37.59±2.67)mmHg,which was lower than(45.24±3.20)mmHg in the control group,the differences between the two groups were statistically significant(P<0.05).Conclusion HFNC can better improve the level of arterial blood gas analysis indexes in BE patients with pulmonary infection,and the complications are less.
