补肾断续汤对胫骨中下段骨折不愈合患者关节功能恢复情况及骨痂评分的影响

Effect of Bushen Duanxu Decoction on Joint Function Recovery and Callus Score in Patients with Non union of Middle and Lower Tibial Fractures

目的探讨补肾断续汤在胫骨中下段骨折不愈合患者中的应用效果。方法选取2021年7月—2023年7月兰陵县中医医院收治的84例胫骨中下段骨折不愈合患者为研究对象,按随机数字表法将其分为对照组和观察组,各42例。对照组采用髓内钉固定术治疗,观察组术后加用补肾断续汤治疗。对比两组患者的关节功能、骨痂评分及骨折愈合时间。结果治疗后,观察组美国纽约特种外科医院中的疼痛评分为(24.27±2.14)分,功能评分为(19.46±1.77)分,活动度评分为(16.74±1.55)分,肌力评分为(8.47±0.95)分,屈曲畸形评分为(7.12±0.55)分,稳定性评分为(8.12±0.50)分,均高于对照组的(21.43±2.51)分、(17.25±1.84)分、(14.42±0.66)分、(7.18±0.46)分、(7.83±0.44)分、(7.24±0.52)分,组间差异有统计学意义(P<0.05);观察组Mazur踝关节症状与功能评分系统评分为(74.22±5.54)分,高于对照组的(65.99±6.11)分,踝关节旋前活动度为(17.43±2.02)°,旋后活动度为(21.53±2.29)°,背伸活动度为(17.53±2.58)°,跖屈活动度为(28.37±3.14)°,均高于对照组的(14.37±2.46)°、(18.19±2.20)°、(14.36±2.01)°、(23.89±3.71)°,组间差异有统计学意义(P<0.05);观察组骨痂评分为(3.13±0.50)分,高于对照组的(2.57±0.47)分,骨折愈合时间为(7.21±1.27)周,短于对照组的(8.94±2.53)周,组间差异有统计学意义(P<0.05)。结论补肾断续汤可以加快胫骨中下段骨折不愈合患者骨折愈合,有利于关节功能恢复。

Objective To investigate the application effect of Bushen Duanxu Decoction in patients with nonunion of middle and lower tibial fractures.Methods Eighty-four patients with non union of middle and lower tibial fractures admitted to Lanling County Hospital of Traditional Chinese Medicine from July 2021 to July 2023 were selected as the research objects,they were divided into a control group and an observation group according to random number table method,with 42 cases each.The control group was treated with intramedullary nail fixation,and the observation group was treated with Bushen Duanxu Decoction.The joint function,callus score and fracture healing time were compared between the two groups.Results After treatment,the pain score in the observation group at the New York Special Surgery Hospital in the United States was(24.27±2.14)points,the function score was(19.46±1.77)points,the motion score was(16.74±1.55)points,the muscle strength score was(8.47±0.95)points,and the flexion deformity score was(7.12±0.55)points,the stability score was(8.12±0.50)points,both were higher than(21.43±2.51)points,(17.25±1.84)points,(14.42±0.66)points,(7.18±0.46)points,(7.83±0.44)points,(7.24±0.52)points of the control group,the differences between groups were statistically significant(P<0.05);the Mazur Ankle Symptom and Function Scoring System score of the observation group was(74.22±5.54)points,which was higher than(65.99±6.11)points of the control group,the ankle pronation motion range was(17.43±2.02)°,supination motion range was(21.53±2.29)°,and dorsoextension motion range was(17.53±2.58)°,the plantar flexor activity was(28.37±3.14)°,both were higher than(14.37±2.46)°,(18.19±2.20)°,(14.36±2.01)°,(23.89±3.71)°of the control group,and the differences between groups were statistically significant(P<0.05);the Callus Score of the observation group was(3.13±0.50)points,which was higher than(2.57±0.47)points of the control group,and the fracture healing time was(7.21±1.27)weeks,which was shorter than(8.94±2.53)weeks of the control group,with statistical significance(P<0.05).Conclusion Bushen Duanxu Decoction can accelerate the fracture healing of patients with non union of middle and lower tibia fractures,and is beneficial to the rehabilitation of joint function.
