重复经颅磁刺激联合虚拟现实训练对脑卒中后上肢偏瘫患者上肢功能及日常生活能力的影响

摘要 目的探讨重复经颅磁刺激联合虚拟现实训练对脑卒中后上肢偏瘫患者的影响。方法选择2020年10月—2023年10月国家电网公司北京电力医院收治的100例脑卒中后上肢偏瘫患者为研究对象,按照随机数字表法将其分为对照组和观察组,各50例。对照组采取常规康复训练,观察组采取重复经颅磁刺激联合虚拟现实训练。比较两组的神经功能、肌张力、上肢功能及日常生活能力。结果干预后,观察组美国国立卫生研究院卒中量表评分低于对照组,差异有统计学意义(P<0.05);观察组MAS等级优于对照组,差异有统计学意义(P<0.05);两组简化Fugl-Meyer上肢运动功能评定表评分、上肢运动力指数(MI)评分均高于干预前,且观察组均高于对照组,差异有统计学意义(P<0.05);两组Barthel指数评分均高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。结论重复经颅磁刺激联合虚拟现实训练可改善脑卒中后上肢偏瘫患者的神经功能和肌张力,可恢复上肢功能及提高日常生活能力。 Objective Exploring the effect of repetitive transcranial magnetic stimulation combined with virtual reality training in patients with upper limb hemiplegia after stroke.Methods Selecting 100 patients with upper limb hemiplegia after stroke admitted to Beijing Electric Power Hospital of State Grid Corporation of China from October 2020 to October 2023 as the research subjects,according to the random number table method,they were divided into a control group and an observation group,with 50 cases in each group.The control group received routine rehabilitation training,while the observation group received repetitive transcranial magnetic stimulation combined with virtual reality training.Compare the neurological function,muscle tone,upper limb function,and daily living ability between two groups.Results After intervention,the National Institutes of Health Stroke Scale score in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05);the MAS grade of the observation group was better than that of the control group,the difference was statistically significant(P<0.05);The scores of the simplified Fugl-Meyer Assessment Upper-Extremity section and Upper Limb Motricity Index in both groups were higher than before the intervention,and the observation group was higher than the control group,the differences were statistically significant(P<0.05);the Barthel Index scores of both groups were higher than before the intervention,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion Repetitive transcranial magnetic stimulation combined with virtual reality training can improve the neurological function and muscle tone of patients with upper limb hemiplegia after stroke,restore upper limb function,and improve ability of daily living.
