舒筋活血汤联合冷疗序贯中药熏蒸对全膝关节置换术后患者膝关节肿胀、疼痛的影响

Effect of Shujin Huoxue Decoction Combined with Cold Therapy Sequential Traditional Chinese Medicine Fumigation on Knee Joint Swelling and Pain in Patients after Total Knee Arthroplasty

目的分析舒筋活血汤联合冷疗序贯中药熏蒸对全膝关节置换术(TKA)后患者的影响。方法选取2021年8月—2023年8月泰安市中医医院收治的76例TKA术后患者为研究对象,按随机数字表法将其分为对照组及观察组,每组38例。对照组采用冷疗序贯中药熏蒸治疗,观察组在对照组基础上采用舒筋活血汤治疗。对比两组患者的临床疗效、膝关节肿胀程度、膝关节疼痛程度、膝关节活动度及膝关节功能。结果观察组的治疗总有效率为94.74%,高于对照组的78.95%,差异有统计学意义(P<0.05)。观察组治疗后膝关节周径与术前的差值为(4.88±0.63)cm,小于对照组的(5.71±1.25)cm,差异有统计学意义(P<0.05)。治疗后,观察组膝关节疼痛视觉模拟评分为(2.26±0.29)分,低于对照组的(3.18±0.37)分,最大伸直角度为(7.51±1.05)°,小于对照组的(10.23±1.64)°,最大屈曲角度为(116.97±9.21)°,大于对照组的(104.39±7.25)°,Lysholm评分为(80.49±6.75)分,高于对照组的(71.36±5.83)分,组间差异有统计学意义(P<0.05)。结论舒筋活血汤联合冷疗序贯中药熏蒸能减轻TKA术后患者的膝关节肿胀程度,减轻其疼痛,增大患者的膝关节活动度,恢复其膝关节功能。

Objective To analyze the effect of Shujin Huoxue Decoction combined with cold therapy sequential traditional Chinese medicine fumigation on patients after total knee arthroplasty(TKA).Methods A total of 76 patients after TKA admitted to Tai'an Traditional Chinese Medicine Hospital from August 2021 to August 2023 were selected as the research objects and were divided into a control group and an observation group according to random number table method,with 38 cases in each group.The control group was treated with cold therapy sequential traditional Chinese medicine fumigation,and the observation group was treated with Shujin Huoxue Decoction on this basis.The clinical efficacy,knee swelling degree,knee pain degree,knee motion and knee function were compared between the two groups.Results The total effective rate of the observation group was 94.74%,which was higher than 78.95%of the control group,the difference was statistically significant(P<0.05).The difference between the knee circumference of the observation group after and before surgery was(4.88±0.63)cm,which was smaller than(5.71±1.25)cm of the control group,and the difference was statistically significant(P<0.05).After treatment,the Visual Analogue Scale of knee pain in the observation group was(2.26±0.29)points,which was lower than(3.18±0.37)points in the control group,the maximum extension angle was(7.51±1.05)°,which was lower than(10.23±1.64)°in the control group,and the maximum flexion angle was(116.97±9.21)°,which was greater than(104.39±7.25)°of the control group,the Lysholm score of the control group was(80.49±6.75)points,which was higher than(71.36±5.83)points of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Shujin Huoxue Decoction combined with cold therapy sequential traditional Chinese medicine fumigation can reduce knee swelling,relieve pain,improve knee motion and restore knee function in patients after TKA.
