PFNA联合术后运动与康复一体化治疗方案在老年股骨转子间骨折患者中的应用效果

Implementation Effect of PFNA Combined with Integrated Therapy for Postoperative Exercise and Rehabilitation in Elderly Patients with Intertrochanteric Fracture

目的分析股骨近端防旋髓内钉(PFNA)联合术后运动与康复一体化治疗方案在老年股骨转子间骨折患者中的应用效果。方法选取本院2019年1月—2023年8月收治的540例接受PFNA治疗的老年股骨转子间骨折患者为研究对象,按随机数字表法将其分成对照组与研究组,各270例。对照组接受术后常规治疗,研究组实施运动与康复一体化治疗,比较两组的康复效果。结果研究组下床活动时间、骨折愈合时间均短于对照组,组间差异有统计学意义(P<0.05)。研究组髋关节功能恢复优良率高于对照组,术后并发症发生率低于对照组,组间差异有统计学意义(P<0.05)。术后6周,研究组疼痛视觉模拟评分低于对照组,差异有统计学意义(P<0.05)。结论老年股骨转子间骨折患者在接受PFNA治疗时,术后采用运动与康复一体化治疗方案能促进其术后及早康复,改善髋关节功能及日常生活能力,且能使术后疼痛程度显著减轻,术后并发症明显减少,值得推广。

Objective To analyze the effect of proximal femoral nail antirotation(PFNA)therapy combined with integrated therapy for postoperative exercise and rehabilitation in elderly patients with intervrochanteric fractures.Methods A total of 540 elderly patients with intertrochanteric fractures treated with PFNA from January 2019 to August 2023 were selected as the research objects.According to the random number table method,they were divided into a control group and a study group,with 270 cases in each group.The control group received postoperative routine treatment,and the study group received exercise and rehabilitation integration treatment.The rehabilitation effects of the two groups were compared.Results The time of getting out of bed and fracture healing in the study group were shorter than those in the control group,and the differences between the two groups were statistically significant(P<0.05).The excellent and good rate of hip joint function recovery in the study group was higher than that in the control group,and the incidence of postoperative complications was lower than that in the control group,and the differences between the two groups were statistically significant(P<0.05).6 weeks after operation,the visual analogue scale score of pain in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion When elderly patients with femoral intertrochanteric fracture received PFNA treatment,the postoperative exercise and rehabilitation integrated treatment program can promote the early rehabilitation of patients after operation,improve hip joint function and daily life ability,and can significantly reduce the degree of postoperative pain and postoperative complications,which is worthy of promotion.
