健脾温肾颗粒对老年维持性血液透析患者疗效及生存状态的影响分析

Effect of Jianpi Wenshen Granules on the Therapeutic Effect and Living Status of Elderly Maintenance Hemodialysis Patients

目的探讨健脾温肾颗粒在老年维持性血液透析患者中的临床效果。方法回顾性分析2023年1—11月泰州市中医院及第二人民医院血液净化中心门诊收治的70例老年维持性血液透析患者的临床资料,根据不同治疗方法将其分为研究组和对照组,各35例。对照组采用常规西医治疗,研究组在对照组的基础上采用健脾温肾颗粒治疗。对比两组患者的生存状态、炎性因子、肾功能、中医症状,以及临床疗效、感染率和30 d内再住院率。结果治疗前,两组患者的营养控制状态评分(CONUT)、炎性因子、肾功能、中医症候积分对比,组间差异无统计学意义(P>0.05)。治疗后,两组CONUT评分均较治疗前降低,且研究组低于对照组,差异有统计学意义(P<0.05);研究组血尿素氮、C反应蛋白、白细胞介素-6水平均低于对照组,血清白蛋白水平高于对照组,组间差异有统计学意义(P<0.05);两组中医症候积分均较治疗前下降,且研究组较对照组低,差异有统计学意义(P<0.05);研究组治疗总有效率高于对照组,感染率和30 d内再住院率均低于对照组,组间差异有统计学意义(P<0.05)。结论健脾温肾颗粒应用到老年维持性血液透析患者治疗过程中,能够促进患者营养状态和炎症状况改善,中医症候显著缓解,治疗总有效率明显提升,同时患者感染及再住院风险较低,能够同时保证治疗有效性和安全性。

Objective To explore the effect of Jianpi Wenshen Granules Elderly Maintenance Hemodialysis Patients.Methods The clinical data of 70 elderly maintenance hemodialysis patients in the Blood Purification Center of Taizhou Hospital of Traditional Chinese Medicine and the Second People's Hospital from January to November 2023 were retrospective analyzed,and they were divided into a research group and a control group according to different treatment methods,with 35 cases in each group.The control group was received conventional Western medicine treatment,and the research group received treatment with Jianpi Wenshen Granules on the basis of the control group.The survival status,inflammatory factors,renal function,TCM symptoms,clinical efficacy,infection rate and re-hospitalization rate within 30 days were compared between the two groups.Results Before treatment,the scores of nutritional control status(CONUT),inflammatory factors,renal function indexes and TCM syndrome scores were compared between the two groups,and the differences between the groups were no statistically significant(P>0.05).After treatment,the CONUT scores in both groups were lower than before treatment,and the research group was lower than that in the control group,and the difference was statistically significant(P<0.05);the levels of blood urea nitrogen,C-reactive protein and interleukin-6 in the treatment group were lower than those in the control group,and the levels of serum albumin was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05);the scores of TCM symptoms in both groups were lower than before treatment,and the research group was lower than that in the control group,and the difference was statistically significant(P<0.05);the total effective rate of the treatment group was higher than that of the control group,the infection rate and the rate of re-hospitalization within 30 days were lower than that of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion The application of Jianpiwenshen granules in the treatment of elderly maintenance hemodialysis patients can improve the nutritional status and inflammation of patients,significantly relieve the symptoms of traditional Chinese medicine,and significantly improve the treatment efficiency.At the same time,the risk of infection and re-hospitalization of patients is low,which can ensure the effectiveness and safety of treatment.
