核心肌群稳定性训练对脑卒中后偏瘫患者步行功能的影响

摘要 目的探讨核心肌群稳定性训练在提高脑卒中后偏瘫患者步行功能中的应用。方法选取广西壮族自治区江滨医院2022年11月—2023年11月收治的84例脑卒中后偏瘫患者为研究对象,按随机数字表法将其分为对照组和观察组,每组42例。对照组采用常规康复干预,观察组在对照组的基础上结合核心肌群稳定性训练干预。比较两组患者的平衡功能、步态分析、运动耐力、运动功能。结果干预后,观察组睁、闭眼状态时运动长度均短于对照组,运动椭圆面积均小于对照组,组间差异有统计学意义(P<0.05);观察组步长长于对照组,步速快于对照组,步宽、步行周期均短于对照组,组间差异有统计学意义(P<0.05);观察组6 min步行试验距离为(182.62±3.43)m,长于对照组的(141.83±3.41)m,Fugl-Meyer运动功能评定量表-下肢部分评分为(23.32±2.45)分,高于对照组的(19.65±2.07)分,组间差异有统计学意义(P<0.05)。结论核心肌群稳定性训练对脑卒中后偏瘫患者的步态有明显的改善作用,还可提高其平衡功能及运动耐力,进而使其运动功能得到提升。 Objective To explore the application of core muscle stability training in improving walking function in patients with hemiplegia after stroke.Methods A total of 84 patients with hemiplegia after stroke admitted to Jiangbin Hospital of Guangxi Zhuang Autonomous Region from November 2022 to November 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 42 cases in each group.The control group received routine rehabilitation intervention,and the observation group combined core muscle stability training intervention on the basis of the control group.Balance function,gait analysis,exercise endurance and exercise function were compared between the two groups.Results After the intervention,in the observation group,the exercise length were shorter than those in the control group,and elliptical area with eyes open and colsed were smaller than those in the control group,the difference was statistically significant(P<0.05);the step length of the observation group was longer than thant in the control group,the step speed was faster than that in the control group,and the step width and walking cycle were shorter than those in the control group,and the differences were statistically significant(P<0.05);the 6-min walking test in the observation group was(182.62±3.43)m,which was longer than(141.83±3.41)m of the control group,and the Fugl-Meyer Motor Function Rating Scale-lower extremity score was(23.32±2.45)points,which was higher than(19.65±2.07)points of the control group,and the differences were statistically significant(P<0.05).Conclusion Core muscle stability training can significantly improve the gait of patients with hemiplegia after stroke,and can also improve their balance function and exercise endurance,so as to improve their exercise function.
