声带矛盾运动障碍诊疗进展

Advances in Diagnosis and Treatment of Paradoxical Vocal Fold Motion

声带矛盾运动障碍(paradoxical vocal fold motion,PVFM)是一种以吸气时声带内收为特征的呼吸系统疾病,也被称为发作性喉部运动障碍、刺激性声带功能障碍、精神性声带功能障碍。其典型表现是突然出现的吸气性喘息,并伴有呼吸困难,无异物吸入、过敏反应或感染的病史。患者常出现喉部紧张感或窒息感,呼吸困难症状常见于吸气过程中,可伴有吸气性喉喘鸣。此外,患者还可出现咳嗽、声音改变等症状。PVFM病因尚不明确,可能与心理疾病、反流性疾病或神经系统疾病有关,目前本病缺乏规范的诊疗流程及共识,本文对PVFM的流行病学、诊断及治疗情况进行综述。

Paradoxical vocal fold motion(PVFM)is a respiratory disorder characterised by inward retraction of the vocal cords during inspiration,a condition also known as episodic laryngeal dyskinesia,irritant vocal cord dysfunction,psychogenic vocal cord dysfunction.The disease is typified by the sudden onset of inspiratory stridor with dyspnoea without a history of foreign body aspiration,allergic reaction or infection.Patients often experience a feeling of laryngeal tightness or choking,and symptoms of dyspnoea are commonly seen during inspiration and may be accompanied by inspiratory laryngeal stridor.In addition,patients may also present with symptoms such as cough and voice changes.The etiology of PVFM is unclear and may be related to psychological disorders,reflux diseases,or neurological disorders.Currently,there is a lack of standardized diagnostic and therapeutic procedures and consensus for this disease.This article provides an overview of the epidemiology,diagnosis,and treatment of PVFM.
