高血压伴认知衰弱老年患者外出障碍调查及影响因素分析

Investigation and analysis of influencing factors of going out disorder in elderly patients with hypertension and cognitive impairment

目的了解高血压伴认知衰弱老年人外出障碍的情况。方法选取本院2020年1月至2023年12月收治的120例高血压伴认知衰弱患者作为调查对象,采用外出障碍量表(OBS)、蒙特利尔认知评估量表(MoCA)以及Fried衰弱评估量表(FFP)进行调查。结果120例患者外出障碍量表得分,外出障碍量表的总平均分为(25.14±2.23)分,得分最高的条目是条目5,平均分为(3.02±0.22)分,其次是条目3,平均分为(3.01±0.34)分,其他条目得分从高到低依次为条目9、条目8、条目6、条目2、条目4、条目1、条目7,分别为(2.98±0.28)分、(2.91±0.32)分、(2.87±0.31)分、(2.77±0.21)分、(2.56±0.17)分、(2.53±0.17)分和(2.49±0.37)分。年龄、高血压病程、跌倒史、认知功能以及衰弱情况等5项在外出障碍量表的得分比较中,差异有统计学意义(P<0.05),多元线性回归分析结果显示,上述5项是高血压伴认知衰弱老年患者外出障碍的影响因素(P<0.05)。结论老年高血压伴认知衰弱人群外出障碍的影响因素主要是有年龄、高血压病程、跌倒史、认知功能以及衰弱情况,针对这些影响因素进行干预,有利于改善患者的外出障碍。

Objective To investigate and analyze the situation of going out disorder in elderly people with hypertension and cognitive impairment.Methods A total of 120 patients with hypertension with cognitive impairment treated in our hospital from January 2020 to December 2023 were investigated with extravagant Disorder scale(OBS),Montreal Cognitive Assessment scale(MoCA)and Fried weakness Assessment scale(FFP).Results The total score of the out disorder scale of 120 patients was(25.14±2.23).The highest item was item 5(3.02±0.22),followed by item 3(3.01±0.34).The other items were 9,8,6,2,4,1 and 7 from high to low.The average scores were(2.98±0.28),(2.91±0.32),(2.87±0.31),(2.77±0.21),(2.56±0.17),(2.53±0.17)and(2.49±0.37)respectively.There were significant differences in age,course of hypertension,history of fall,cognitive function and weakness in the scores of excursion disorder scale(P<0.05).Age,history of fall,course of hypertension,cognitive function and weakness were the influencing factors of going out disorder in elderly hypertensive patients with cognitive impairment(P<0.05).Conclusion The main influencing factors of going out disorder in elderly patients with hypertension with cognitive impairment are age,course of hypertension,history of fall,cognitive function and weakness.Intervention against these factors is helpful to improve the patients going out disorder.
