自我决定理论下的护理干预在老年高血压患者中的应用

Application of nursing intervention based on self-determination theory in elderly hypertensive patients

目的探讨基于自我决定理论的护理干预在老年高血压患者中的护理疗效。方法将2023年11月至2024年7月在上海中医药大学附属曙光医院老年医学科诊治的160例老年高血压患者随机分成试验组与对照组,各80例,观察随访3个月。对照组给予常规护理,试验组实施基于自我决定理论的护理干预,对比两组的心理需要满足程度、高血压患者自我管理行为能力、服药依从性、血压及护理满意度。结果干预后,两组的基本心理需求量表(BPNS)评分均高于干预前,且试验组显著高于对照组(P<0.01);两组的自我管理行为能力评分都升高,且试验组高于对照组,有统计学差异(P<0.05);试验组的服药依从性显著高于对照组(P<0.01);试验组的平均动脉压水平低于对照组,有统计学差异(P<0.05);患者对试验组的护理满意度显著高于对照组(P<0.01)。结论对老年高血压患者应用自我决定理论下的护理干预,能提升患者的心理需要满足程度和自我管理行为能力,从而提高护理疗效。

Objective To analyze the nursing effect of nursing intervention based on self-determination theory(SDT)on elderly hypertensive patients.Methods A total of 160 elderly hypertensive patients admitted to department of geratology,Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine from November 2023 to July 2024 were selected as the study subjects,with a three-month course of Observation and follow-up.They were divided into control group and study group,and each group enrolled 80 cases.The control group received routine continuous nursing,and the study group received nursing intervention based on SDT.Their psychological needs satisfaction levels,hypertension patient self-management behavior abilities,medication compliance,blood pressure levels and nursing satisfaction were compared between the two groups.Results After intervention,the basic psychological needs scale(BPNS)scores of the two groups were higher than before,and the scores of the study group were significantly higher than those of the control group(P<0.01).The hypertension patient self-management behavior rating scale(HPSMBRS)scores of the two groups were higher than before,and the scores of the study group were higher than those of the control group(P<0.05).The medication compliance of the study group was significantly higher than that of the control group(P<0.01).The mean arterial pressure of the study group were lower than those of the control group(P<0.05).The nursing satisfaction of the study group was significantly higher than that of the control group(P<0.01).Conclusion The application of nursing based on self-determination theory in elderly hypertensive patients can effectively elevate their psychological needs satisfaction levels and the self-management behavior abilities of hypertension patients,thereby improve the nursing effect.
