老年骨质疏松性骨折患者疾病感知及康复需求的质性研究

Qualitative study on disease perception and rehabilitation needs of elderly patients with osteoporotic fractures

目的探讨老年骨质疏松性骨折患者疾病感知及康复需求,为提高患者生活质量提供参考。方法采用目标抽样法对符合标准的老年骨质疏松性骨折患者进行半结构式访谈,并选用Colaizzi分析法对访谈内容分析。结果共提炼出4个核心主题及10个亚主题:躯体感知(躯体不适、行动不便)、心理感知(感到脆弱、对家庭愧疚、患病羞耻感)、应对方式(重拾信心、生活失望)、康复需求(过渡期护理需求、中医护理服务需求、“互联网+中医护理服务”需求)。结论老年骨质疏松性骨折患者疾病感知、应对方式及康复需求较丰富,可通过改善老年骨折患者躯体及心理感知,满足患者的康复需求,来提高骨折患者生活质量。

Objective To explore the disease perception and needs of elderly patients with osteoporotic fractures,and provide reference for alleviating physical and mental discomfort,meeting personalized needs,and improving quality of life.Methods Semi structured interviews were conducted with eligible elderly patients with osteoporotic fractures,and Colaizzi analysis was used to analyze the interview content.Results A total of 4 core themes and 10 sub themes were extracted:somatic perception(physical discomfort,mobility inconvenience),psychological perception(feeling vulnerable,guilty of family,feeling ashamed of illness),coping style(regaining confidence,life disappointment),rehabilitation needs(transitional care needs,Chinese medicine care services needs,“Internet plus Chinese medicine care services”needs).Conclusion Elderly patients with osteoporotic fractures have rich disease perception,coping strategies,and rehabilitation needs.Improving the physical and psychological discomfort of elderly fracture patients and meeting their rehabilitation needs can improve their quality of life.
