帕罗西汀联合奥氮平治疗精神分裂症稳定期伴焦虑效果观察

Effect of paroxetine combined with olanzapine in the treatment of stable schizophrenia with anxiety

目的 研究帕罗西汀联合奥氮平治疗精神分裂症(SCH)稳定期伴焦虑的效果。方法 选择东莞市第七人民医院2021年10月至2023年3月收治的SCH稳定期伴焦虑患者70例为研究对象,按随机数表法分为对照组(复合维生素联合奥氮平)、观察组(帕罗西汀联合奥氮平),每组各35例。两组均持续治疗12周。比较两组临床疗效、阳性和阴性症状[阳性和阴性症状量表评分表(PANSS)]、焦虑状态[焦虑自评量表(SAS)]、病耻感、缺陷感[缺陷感量表(FIS)]及不良反应发生情况。结果 观察组临床总有效率(94.29%)高于对照组(74.29%),治疗后PANSS各维度、SAS、精神病患者病耻感评估量表、FIS评分均低于对照组,差异有统计学意义(P <0.05);两组不良反应总发生率相比,差异无统计学意义(P> 0.05)。结论 帕罗西汀联合奥氮平治疗SCH稳定期伴焦虑的效果显著,可有效改善其阳性和阴性症状,减轻焦虑状态,改善病耻感与缺陷感,且不会增加不良反应的发生,具有较好的应用安全性。

Objective Study on the efficacy of paroxetine combined with olanzapine in the treatment of stable schizophrenia(SCH)with anxiety.Methods A total of 70 patients with schizophrenia in stable stage with anxiety admitted to Dongguan Seventh People’s Hospital from October 2021 to March 2023 were selected as the study subjects,and were divided into two groups using a random number table method,with 35 patients in each group.The control group(35 cases)was treated with multivitamin combined with olanzapine,and the observation group(35 cases)was treated with paroxetine combined with olanzapine.Both groups received continuous treatment for 12 weeks.Compare the clinical efficacy,positive and negative symptoms(PANSS),self rating anxiety scale(SAS),shame,the feelings of inadequacy scale(FIS),and incidence of adverse reactions between two groups.Results The total clinical effective rate of the observation group(94.29%)was higher than that of the control group(74.29%).After treatment,the scores of PANSS dimensions,SAS,psychiatric shame assessment scale,and FIS scale were all lower than those of the control group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Paroxetine combined with olanzapine has a significant therapeutic effect on stable SCH with anxiety,effectively improving its positive and negative symptoms,alleviating anxiety,improving shame and deficiency,and does not increase the occurrence of adverse reactions.It has good application safety.
