肿瘤相关淋巴水肿患者疾病感知与希望水平相关性研究

A study on the correlation between illness perception and hope level in patients with tumor-associated lymphedema

目的 探讨肿瘤相关淋巴水肿患者疾病感知与希望水平的影响因素及其之间的相关性,为提高患者疾病感知及希望水平提供参考。方法 选取2022年10月至2023年5月福建省肿瘤医院100例符合纳入排除标准的淋巴水肿患者为研究对象,采用疾病感知问卷简化版(BIPQ)和Herth希望量表(HHI)进行调查分析。结果 肿瘤相关淋巴水肿患者BIPQ得分为(46.66±10.71)分,HHI得分为(33.57±5.47)分,处于中高水平。多元线性回归分析结果显示文化程度、水肿分期和水肿严重程度是疾病感知的主要影响因素(P <0.05),水肿分期、水肿严重程度、水肿持续时间影响患者希望水平(P <0.05);Pearson相关性分析结果显示疾病感知中认知表征维度、疾病理解能力维度及BIPQ总分分别与希望水平中积极的态度维度、积极的行动维度、亲密的关系维度及总分呈负相关(P <0.01)。结论 护理人员在对肿瘤相关淋巴水肿患者进行护理时应更加关注文化程度低、水肿严重且持续时间长的患者,提供针对性的健康教育,通过提高患者的积极疾病感知,来提高其希望水平。

Objective To explore the influencing factors of illness perception and hope level and the correlation between them in patients with tumor-associated lymphedema,so as to provide reference for improving patients’disease perception and hope level.Methods A total of 100 patients with lymphedema who met the inclusion and exclusion criteria from Fujian Cancer Hospital form October 2022 to May 2023 were selected as the study subjects.The brief illness perception questionnaire(BIPQ)and Herth hope index(HHI)were used for investigation and analysis.Results The score of BIPQ and HHI in patients with tumor-associated lymphedema was(46.66±10.71)and(33.57±5.47),which were in the middle and high level.The results of multiple linear regression analysis showed that education level,edema stage and edema severity were the main influencing factors of illness perception(P<0.05),while edema stage,edema severity and edema duration affected patients’hope level(P<0.05).The Pearson correlation analysis results showed that the cognitive representation dimension,illness understanding ability dimension,and BIPQ total score in illness perception were negatively correlated with the positive attitude dimension,positive action dimension,intimate relationship dimension,and total score in hope level,respectively(P<0.01).Conclusion Nursing staff should pay more attention to patients with tumor-associated lymphedema who have low educational levels,severe edema,and a long duration when caring for them.Targeted health education should be provided to improve the patient’s positive illness perception and hope level.
