中医特色护理联合渐进式干预对骨质疏松性椎体压缩骨折患者恢复的影响

Effect of traditional Chinese medicine specialized nursing combined with progressive intervention on the recovery of osteoporotic vertebral compression fractures

目的探讨中医特色护理联合渐进式干预对骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fracture,OVCF)恢复的影响。方法选取2021年1月至2023年12月天津市滨海新区海滨人民医院收治的100例OVCF患者,采用随机数字表分为研究组(50例)与对照组(50例),两组均给予常规干预,在此基础上对照组给予渐进式干预,研究组给予中医特色护理联合渐进式干预,均连续干预1个月。比较两组临床症状改善情况(疼痛消失、肿胀消失及骨折愈合时间),比较两组干预前后腰椎骨密度(BMD)、腰背部功能[Oswestry功能障碍指数问卷表(ODI)]及生活质量[生活质量综合评定问卷(GQOL-74)]改善情况,比较两组二次骨折发生情况。结果研究组疼痛消失、肿胀消失及骨折愈合时间分别为(10.34±2.17)d、(12.62±2.40)d、(9.86±1.77)周均短于对照组(12.14±2.32)d、(15.38±2.69)d、(12.74±1.82)周(P<0.05);干预后两组BMD T值均高于干预前,ODI评分低于干预前,且研究组BMD T值高于对照组,ODI评分低于对照组(P<0.05);干预后两组躯体功能、社会功能、心理功能及物质生活均高于干预前,且研究组上述维度评分高于对照组(P<0.05);研究组二次骨折发生率低于对照组(P<0.05)。结论中医特色护理联合渐进式干预可促进OVCF患者恢复,改善其骨密度及腰背部功能,提高生活质量,降低二次骨折发生风险。

Objective To explore the effect of traditional Chinese medicine(TCM)specialized nursing combined with progressive intervention on the recovery of osteoporotic vertebral compression fractures(OVCF).Methods A total of 100 OVCF patients admitted to Haibin People's Hospital of Tianjin Binhai New Area from January 2021 to December 2023 were randomly divided into the observation group(50 cases)and study group(50 cases)using a random number table method.Patients in both groups received routine intervention and progressive intervention,and those in the observation group additionally received TCM specialized nursing.A continuous intervention was performed for one month.The improvement of clinical symptoms(pain disappearance,swelling disappearance and fracture healing time),lumbar bone mineral density(BMD),lumbar and back function(the Oswestry disability index questionnaire[ODI])and quality of life(the generic quality of life inventory 74[GQOL-74])before and after intervention were compared between the two groups,and the occurrence of secondary fractures was compared between the two groups.Results The disappearance of pain([10.34±2.17]d VS[12.14±2.32]d),swelling([12.62±2.40]d VS[15.38±2.69]d),and fracture healing time([9.86±1.77]weeks VS[12.74±1.82]weeks)in the observation group were significantly shorter than those of the study group(P<0.05).After intervention,the BMD T values of both groups were higher than those before intervention,and the ODI score was lower than before intervention,which were significantly pronounced in the observation group than in the study group(P<0.05).After the intervention,the physical function,social function,psychological function,and material life of the two groups were all higher than those before the intervention,and the scores of the above dimensions in the observation group were significantly higher than those in the study group(P<0.05).The incidence of secondary fractures in the observation group was significantly lower than that of the study group(P<0.05).Conclusion TCM specialized nursing combined with progressive intervention can promote the recovery of OVCF,improve the bone mineral density,lumbar back function and quality of life,and reduce the risk of secondary fractures.
