疾病诊断相关分组背景下基于PDCA的住院病案首页管理与实践效果

Effectiveness of PDCA-based Inpatient Case Home Page Management and Practice in the Context of Disease Diagnosis-related Grouping

目的:分析疾病诊断相关分组(DRG)背景下基于PDCA的住院病案首页管理与实践效果。方法:选取2022年1—6月无锡市第二人民医院尚未开展PDCA管理期间住院病案首页100份(实施前),另选取2022年7—12月在DRG背景下开展PDCA管理期间住院病案首页100份(实施后)。对比实施前后主要诊断正确率、主要手术及操作正确率、其他诊断正确率、病案首页完整率、DRG入组率。结果:实施后主要诊断正确率、主要手术及操作正确率、其他诊断正确率、病案首页完整率、DRG入组率均高于实施前,差异有统计学意义(P<0.05)。结论:DRG背景下基于PDCA的住院病案首页管理与实践效果较好,可以提高主要诊断正确率、主要手术及操作正确率、其他诊断正确率、病案首页完整率、DRG入组率。

Objective:To analyze the effect of inpatient case home page management and practice based on PDCA in the context of disease diagnosis-related grouping(DRG).Methods:One hundred inpatient case home pages of Wuxi NO.2 People's Hospital during the period of January to June 2022,when PDCA management had not yet been carried out(before implementation),and another one hundred inpatient case home pages of our hospital during the period of July to December 2022,when PDCA management had been carried out in the context of DRG(after implementation),were selected.Compared the correct rate of main diagnosis,correct rate of main surgery and operation,correct rate of other diagnosis,complete rate of case home page,and DRG enrollment rate before and after implementation.Results:The rates of correct major diagnosis,correct major surgery and operation,correct other diagnosis,complete case home page,and DRG enrollment were higher after implementation than before implementation,the differences were statistically significant(P<0.05).Conclusion:PDCA-based management and practice of inpatient case home page in the context of DRG is more effective and can improve the rate of correct major diagnosis,correct major surgery and operation,correct other diagnosis,complete case home page,and DRG admission rate.
