螺旋CT联合血清Pro GRP、NSE、CEA、CYFRA21-1在临床早期诊断肺癌中的应用价值

The Application Value of Spiral CT Combined with Serum ProGRP,NSE,CEA,and CYFRA21-1 in the Early Diagnosis of Lung Cancer in Clinical Practice

目的分析螺旋CT联合血清胃泌素释放肽前体(Pro GRP)、神经元特异性烯醇化酶(NSE)、癌胚抗原(CEA)、细胞角质蛋白2-11(CYFRA21-1)在临床早期诊断肺癌中的应用价值。方法选取2021年1月至2023年至1月于医院就诊的80例早期肺癌患者,纳入早期肺癌组,同期80例良性肺部病变患者,纳入良性组。比较两组资料,包括螺旋CT检查结果和多种肿瘤标志物检测结果,分析螺旋CT联合血清Pro GRP、NSE、CEA、CYFRA21-1在临床早期诊断肺癌中的应用价值。结果螺旋CT诊断早期肺癌的灵敏度为87.50%(70/80)、特异度为86.25%(69/80)、准确度为86.88%(139/160)、阴性预测值为87.34%(69/79)、阳性预测值为86.42%(70/81);早期肺癌血清Pro GRP、NSE、CEA、CYFA21-1高于良性组,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,螺旋CT和血清肿瘤标志物Pro GRP、NSE、CEA、CYFA21-1均与早期肺癌有关(P<0.05);绘制ROC曲线发现,螺旋CT检查结果和血清Pro GRP、NSE、CEA、CYFA21-1诊断早期肺癌的AUC分别为0.869、0.907、0.820、0.900、0.784,均有一定诊断价值,联合诊断回归模型诊断早期肺癌的AUC为0.989,>0.80,诊断价值高,敏感度、特异度分别为0.975、0.988。结论螺旋CT联合血清Pro GRP、NSE、CEA、CYFRA21-1在临床早期诊断肺癌中的应用价值较高。

Objective To analyze the value of spiral CT combined with serum Gastrin releasing peptide precursor(ProGRP),neuron specific enolase(NSE),carcinoembryonic antigen(CEA),and cytokeratin 2-11(CYFRA21-1)in the early clinical diagnosis of lung cancer.Methods 80 patients with early lung cancer who visited hospitals from January 2021 to January 2023 were selected and included in the early lung cancer group.During the same period,80 patients with benign lung lesions were included in the benign group.Compare two sets of data,including spiral CT examination results and multiple tumor marker detection results,and analyze the application value of spiral CT combined with serum ProGRP,NSE,CEA,and CYFRA21-1 in the early clinical diagnosis of lung cancer.Results The sensitivity of spiral CT in diagnosing early lung cancer was 87.50%(70/80),specificity was 86.25%(69/80),accuracy was 86.88%(139/160),negative predictive value was 87.34%(69/79),and positive predictive value was 86.42%(70/81);the serum levels of ProGRP,NSE,CEA,and CYFA21-1 in early lung cancer were higher than those in the benign group,with statistical significance(P<0.05);the results of logistic regression analysis showed that spiral CT and serum tumor markers ProGRP,NSE,CEA,and CYFA21-1 were all associated with early lung cancer(P<0.05);drawing the ROC curve,it was found that the results of spiral CT examination and the AUC of serum ProGRP,NSE,CEA,and CYFA21-1 for diagnosing early lung cancer were 0.869,0.907,0.820,0.900,and 0.784,respectively,with certain diagnostic value.The AUC of the combined diagnostic regression model for diagnosing early lung cancer was 0.989,>0.80,with high diagnostic value,sensitivity,and specificity of 0.975 and 0.988,respectively.Conclusion The combination of spiral CT and serum ProGRP,NSE,CEA,and CYFRA21-1 has high clinical value in the early diagnosis of lung cancer.
