名中医林家坤学术思想“通阳法”治疗气虚血瘀型痛性糖尿病周围神经病变临床观察

Clinical Observation on the Treatment of Painful Diabetes Peripheral Neuropathy of Qi Deficiency and Blood Stasis Type by Lin Jiakun,A Famous Doctor of Chinese Medicine

目的探讨气虚血瘀型痛性糖尿病周围神经病变(PDPN)患者采用名中医林家坤学术思想“通阳法”治疗的效果。方法选择2021年7月至2023年5月医院收治的PDPN患者80例,使用抛掷硬币法分为2组,各40例,均实施常规治疗,对照组采用甲钴胺片治疗,基于此,观察组加用通阳活络颗粒,比较两组治疗效果、神经传导速度(NCV)及血糖水平。结果较对照组,观察组治疗总有效率更高(P<0.05);治疗后,两组神经传导速度(SCV)、运动神经传导速度(MCV)均比治疗前高,且观察组比对照组高(P<0.05);两组治疗后空腹血糖(FPG)、糖化血红蛋白(HbA1C)及餐后2h血糖(2hPG)水平均降低,且观察组更低(P<0.05)。结论气虚血瘀型PDPN患者采用通阳活络颗粒治疗效果满意,可提高NCV,调节血糖水平。

Objective To explore the therapeutic effect of Lin Jiakun,a famous Chinese medicine,on painful diabetes peripheral neuropathy(PDPN)patients with qi deficiency and blood stasis,who were treated with the"Tongyang Method".Methods 80 patients with PDPN admitted to the hospital from July 2021 to May 2023 were selected and divided into two groups using the coin toss method,with 40 patients in each group receiving conventional treatment.The control group was treated with methylcobalamin tablets,while the observation group was treated with Tongyang Huoluo granules.The treatment effects,nerve conduction velocity(NCV),and blood glucose levels of the two groups were compared.Results Compared with the control group,the observation group had a higher total effective rate of treatment(P<0.05);After treatment,the nerve conduction velocity(SCV)and motor nerve conduction velocity(MCV)in both groups were higher than before treatment,and the observation group was higher than the control group(P<0.05);After treatment,both groups showed a decrease in fasting plasma glucose(FPG),glycated hemoglobin(HbA1C),and 2-hour postprandial plasma glucose(2hPG)levels,with the observation group showing even lower levels(P<0.05).Conclusion The treatment effect of Tongyang Huoluo Granules on PDPN patients with qi deficiency and blood stasis is satisfactory,which can improve NCV and regulate blood glucose levels.
