2型糖尿病肾病患者血糖联合肾小球滤过率、尿微量白蛋白检测的意义分析

摘要 目的探讨血糖、肾小球滤过率联合尿微量白蛋白对2型糖尿病肾病的诊断价值。方法选取2023年1—12月淄博市妇幼保健院收治的50例2型糖尿病肾病患者作为观察组,同期50名健康体检人群作为对照组。两组均进行空腹血糖、餐后2 h血糖、糖化血红蛋白、肾小球滤过率及尿微量白蛋白检测,比较两组各指标水平,并分析指标单一和联合检测的诊断效能。结果观察组空腹血糖、餐后2 h血糖、糖化血红蛋白、尿微量白蛋白水平均高于对照组,肾小球滤过率低于对照组,差异有统计学意义(P均<0.05)。空腹血糖、餐后2 h血糖、糖化血红蛋白、肾小球滤过率、尿微量白蛋白联合检测的曲线下面积高于空腹血糖+肾小球滤过率、糖化血红蛋白+肾小球滤过率、餐后2 h血糖+尿微量白蛋白、糖化血红蛋白+尿微量白蛋白检测。结论2型糖尿病肾病患者空腹血糖、餐后2 h血糖、糖化血红蛋白及尿微量白蛋白水平明显升高,肾小球滤过率降低,联合检测的诊断效能更高。 Objective To investigate the diagnostic value of blood glucose,glomerular filtration rate combined with urinary microalbumin in type 2 diabetic nephropathy.Methods A total of 50 patients with type 2 diabetic nephropa⁃thy admitted to Zibo Maternal and Child Health Hospital from January to December 2023 were selected as the observa⁃tion group,and 50 healthy physical examination people during the same period were selected as the control group.Fasting plasma glucose,2-hour postprandial plasma glucose,glycosylated hemoglobin A1c,glomerular filtration rate and urinary microalbumin were detected in the two groups,the levels of each index were compared between the two groups,and the diagnostic efficacy of single and combined detection of indexes were analyzed.Results The levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c and urinary microalbumin in the observation group were higher than those in the control group,and the glomerular filtration rate was lower than that in the control group,and the differences were statistically significant(all P<0.05).The area under the curve of combined detection of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c,glomeru⁃lar filtration rate and urinary microalbumin was higher than that of fasting plasma glucose+glomerular filtration rate,glycated hemoglobin A1c+glomerular filtration rate,2-hour postprandial plasma glucose+urinary microalbumin,gly⁃cated hemoglobin A1c+urinary microalbumin.Conclusion The levels of fasting plasma glucose,2-hour postprandial plasm a glucose,glycated hemoglobin A1c and urinary microalbumin in patients with type 2 diabetic nephropathy were significantly increased,the glomerular filtration rate was decreased,and the diagnostic efficiency of combined de⁃tection was higher.
