右美托咪定复合丙泊酚在乳腺外科良性包块切除术中的麻醉效果及安全性分析

Anesthetic Effect and Safety Analysis of Dexmedetomidine Combined with Propofol in Benign Mass Resection of Breast Surgery

目的研究右美托咪定复合丙泊酚在乳腺外科良性包块切除术中的麻醉效果及安全性。方法选取2019年3月-2022年3月在我院接诊的70例在乳腺外科良性包块切除术患者为研究对象,采用随机数字表法分为对照组(n=35)和观察组(n=35)。对照组采用丙泊酚麻醉,观察组在对照组基础上复合右美托咪定。比较两组麻醉指标、疼痛(VAS)评分、Ramsay镇静评分、不同时间段血流动力学指标[动脉压(MAP)、心率(HR)、呼吸频率(RR)]、不良反应发生率。结果观察组苏醒时间、意识恢复时间、拔管时间均短于对照组(P<0.05);观察组术后2、8 h的VAS评分均小于对照组(P<0.05);观察组Ramsay镇静评分高于对照组(P<0.05);两组MAP、HR、RR均高于插管前,但观察组低于对照组(P<0.05);观察组不良反应发生率为5.71%,与对照组的8.57%比较,差异无统计学意义(P>0.05)。结论右美托咪定复合丙泊酚应用于乳腺外科良性包块切除术中麻醉效果理想,苏醒、意识恢复时间短,VAS评分、Ramsay镇静评分高,且对血流动力学影响小,不增加不良反应,具有良好的有效性和安全性。

Objective To study the anesthetic effect and safety of dexmedetomidine combined with propofol in benign mass resection of breast surgery.Methods A total of 70 patients with benign mass resection in breast surgery who were admitted to our hospital from March 2019 to March 2022 were selected as the research objects.They were divided into control group(n=35)and observation group(n=35)by random number table method.The control group was anesthetized with propofol,and the observation group was combined with dexmedetomidine on the basis of the control group.Anesthesia indexes,pain score(VAS),Ramsay sedation score,hemodynamic indexes[arterial pressure(MAP),heart rate(HR),respiratory rate(RR)]at different time periods and incidence of adverse reactions were compared between the two groups.Results The recovery time,consciousness recovery time and extubation time of the observation group were shorter than those of the control group(P<0.05).The VAS score of the observation group at 2 and 8 h after operation was lower than that of the control group(P<0.05).The Ramsay sedation score of the observation group was higher than that of the control group(P<0.05).The MAP,HR and RR of the two groups were higher than those before intubation,but those in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was 5.71%,compared with 8.57% in the control group,but the difference was not statistically significant(P>0.05).Conclusion Dexmedetomidine combined with propofol is effective in anesthesia for benign mass resection in breast surgery,the recovery time and consciousness recovery time are short,the VAS score and Ramsay sedation score are high.Meanwhile,it has little effect on hemodynamics and does not increase adverse reactions.It has good efficacy and safety.
