双腔支气管麻醉在基层医院胸外科手术中的麻醉效果

Anesthetic Effect of Double Lumen Bronchial Anesthesia in Thoracic Surgery in Primary Hospital

目的:探究双腔支气管麻醉在基层医院胸外科手术患者中的应用效果。方法:选取在邳州市中医院行胸外科手术的患者86例(2021年1月—2023年12月),以随机数字表法分为对照组(单腔气管插管麻醉)与观察组(双腔支气管麻醉),各43例。对比两组麻醉效果、平均动脉压及心率、血清指标及并发症。结果:与对照组相比,观察组麻醉深度更大,意识消失时间、自主呼吸恢复时间更短(P<0.05);术后,两组平均动脉压、心率均升高,观察组低于对照组(P<0.05);术后,两组血清肺表面活性蛋白A(SP-A)、肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)水平均高于术前,观察组SP-A、TNF-α、IL-6水平低于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.024)。结论:将双腔支气管应用于基层医院胸外科手术麻醉患者中,可有效提高患者麻醉效果,改善血清指标,使患者生命体征更加平稳,且并发症较少。

Objective:To explore the application effect of double lumen bronchial anesthesia in thoracic surgery patients in primary hospitals.Methods:A total of 86 patients undergoing thoracic surgery in Pizhou Hospital of Traditional Chinese Medicine(January 2021 to December 2023)were selected and divided into control group(single lumen tracheal intubation anesthesia)and observation group(double lumen bronchial anesthesia)by random number table method,with 43 cases each.The anesthetic effect,mean arterial pressure and heart rate,serum indexes and complications were compared between the two groups.Results:Compared with the control group,the depth of anesthesia was higher,the time of consciousness loss and recovery time of spontaneous respiration were shorter in the observation group(P<0.05);after operation,the mean arterial pressure and heart rate of both groups were increased,and the observation group was lower than the control group(P<0.05);after surgery,the levels of serum pulmonary surface active protein A(SP-A),tumor necrosis factor(TNF-α)and interleukin-6(IL-6)in two groups were higher than before surgery,and the observation group's SP-A,TNF-αand IL-6 levels were lower than the control group(P<0.05);the observation group's complication rate was lower than the control group(P=0.024).Conclusion:The application of double lumen bronchus in the anesthesia of thoracic surgery patients in primary hospitals can effectively improve the anesthetic effect of patients,improve serum indexes,make patients'vital signs more stable,and fewer complications.
