不同剂量米非司酮联合曲普瑞林治疗子宫肌瘤的临床效果比较

Comparison of Clinical Effects of Different Doses of Mifepristone Combined with Triptorelin in the Treatment of Uterine Fibroids

目的:分析不同剂量米非司酮联合曲普瑞林治疗子宫肌瘤的临床效果。方法:选取2022年5月—2023年5月贵航贵阳医院妇科收治的子宫肌瘤患者80例为研究对象,以奇偶数法随机分为对照组和研究组,各40例。对照组行常规剂量米非司酮联合曲普瑞林治疗,研究组采取低剂量米非司酮联合曲普瑞林治疗。比较两组性激素指标、子宫肌瘤体积、子宫体积、月经情况、不良反应发生率及复发率。结果:治疗后,两组雌二醇、促黄体生成素、孕酮、促卵泡激素水平均下降,研究组低于对照组(P<0.05);治疗后30、90 d,两组子宫肌瘤体积、子宫体积均小于治疗前,月经周期长于治疗前,月经量少于治疗前,且研究组优于对照组(P<0.05);研究组不良反应发生率低于对照组(P=0.0253);治疗后2、4、6个月,两组子宫肌瘤复发率比较,差异无统计学意义(P>0.05)。结论:低剂量米非司酮联合曲普瑞林治疗子宫肌瘤的效果更优,可以改善患者性激素水平及子宫肌瘤体积、子宫体积、月经情况,降低不良反应发生率,且不会增加复发率。

Objective:To analyze the clinical effects of different doses of mifepristone combined with triptorelin in the treatment of uterine fibroids.Methods:Eighty patients with uterine fibroids admitted to the Department of Gynecology of Guihang Guiyang Hospital from May 2022 to May 2023 were selected as the study subjects,and were randomly divided into the control group and the study group by the odd-even method,each with forty cases.The control group was treated with conventional-dose mifepristone combined with triptorelin,and the study group was treated with low-dose mifepristone combined with triptorelin.The two groups were compared in terms of sex hormone indexes,fibroid volume,uterine volume,menstruation,incidence of adverse reactions and recurrence rate.Results:After treatment,the levels of estradiol,luteinizing hormone,progesterone,and follicle-stimulating hormone decreased in both groups,and the study group was lower than the control group(P<0.05);at 30 and 90 d after treatment,the volume of uterine fibroids and uterine volume in both groups was smaller than that before treatment,the menstrual cycle was longer than that before treatment,and the amount of menstruation was less than that before treatment,and the study group was better than the control group(P<0.05);the incidence of adverse reactions in the study group was lower than that in the control group(P=0.0253);the recurrence rate of uterine fibroids in the two groups was compared at 2,4 and 6 months after treatment,and the difference was not statistically significant(P>0.05).Conclusion:Low-dose mifepristone combined with triptorelin is more effective in the treatment of uterine fibroids,which can improve the patients'sex hormone level and uterine fibroid volume,uterine volume,menstruation,reduce the incidence of adverse reactions,and does not increase the recurrence rate.
