甘精胰岛素联合阿卡波糖对老年糖尿病患者肠道激素的影响及不良反应探讨

Effects and Adverse Reactions of Insulin Glargine Combined with Acarbose on Intestinal Hormones in Elderly Patients with Diabetes

目的:探究甘精胰岛素联合阿卡波糖治疗老年糖尿病(DM)的临床效果。方法:选取商洛市商州区人民医院2022年1月—2023年12月收治的86例老年DM患者作为研究对象,依据随机数字表法分为对照组(阿卡波糖)和观察组(阿卡波糖联合甘精胰岛素),各43例。比较两组血糖水平、肠道激素水平、不良反应发生率。结果:治疗后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均降低,观察组低于对照组(P<0.05);治疗后,两组血清抑胃肽水平均降低,胰高血糖素样肽-1、空腹C肽水平均升高,观察组优于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05);观察组总有效率高于对照组(P=0.024)。结论:甘精胰岛素联合阿卡波糖治疗老年DM的效果较好,可有效改善血糖及肠道激素水平,且不会增加不良反应。

Objective:To explore the clinical effect of insulin glargine combined with acarbose in the treatment of senile diabetes mellitus(DM).Methods:A total of eighty-six elderly DM patients admitted to Shangluo Shangzhou District People's Hospital from January 2022 to December 2023 were selected as the study objects,and were divided into control group(acarbose)and observation group(acarbose combined with insulin glargine)according to random number table method,with forty-three cases in each group.Blood glucose level,intestinal hormone level and incidence of adverse reactions were compared between the two groups.Results:After treatment,the levels of fasting blood glucose,2 h postprandial blood glucose and glycosylated hemoglobin in both groups were decreased,and the observation group was lower than the control group(P<0.05);after treatment,the serum gastric inhibitory peptide levels were decreased in both groups,and the levels of glucagon-like peptide-1 and fasting C-peptide were increased,and the observation group was better than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05);the total effective rate of observation group was higher than that of control group(P=0.024).Conclusion:Insulin glargine combined with acarbose is effective in the treatment of elderly DM,which can effectively improve blood glucose and intestinal hormone levels without increasing adverse reactions.
