超声引导下选择性颈神经根阻滞治疗混合型颈椎病的临床观察

Clinical Observation of Ultrasound-guided Selective Cervical Nerve Root Block in the Treatment of Mixed Cervical Spondylosis

目的:探究超声引导下选择性颈神经根阻滞治疗混合型颈椎病的临床效果。方法:选取2023年4月—2024年5月贵阳市第二人民医院收治的70例混合型颈椎病患者为研究对象,按照随机数字法分为对照组(传统阻滞疗法)和观察组(超声引导下选择性颈神经根阻滞治疗),各35例。对比两组治疗效果、疼痛评分、颈椎功能、日常生活能力、不良反应发生率。结果:观察组治疗总有效率高于对照组(P=0.012);治疗后3个月,两组疼痛分级指数、疼痛视觉量表、疼痛程度评分及简版McGill疼痛问卷总分下降,且观察组低于对照组(P<0.05);治疗后3个月,两组颈椎功能障碍指数量表评分下降,Barthel指数升高,且观察组优于对照组(P<0.05);两组不良反应发生率对比,无统计学差异(P>0.05)。结论:超声引导下选择性颈神经根阻滞治疗混合型颈椎病的效果较好,可有效减轻疼痛,改善颈椎功能、日常生活能力,安全性较高。

Objective:To explore the clinical effect of ultrasound-guided selective cervical nerve root block in the treatment of mixed cervical spondylosis.Methods:Seventy patients with mixed cervical spondylosis treated in Guiyang Second People's Hospital from April 2023 to May 2024 were selected as the study objects,and were divided into control group(traditional block therapy)and observation group(ultrasound-guided selective cervical nerve root block therapy)according to random number method,with thirty-five cases in each group.The treatment effect,pain score,cervical spine function,ability of daily living and incidence of adverse reactions were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.012);three months after treatment,the pain grading index,pain visual scale,pain degree score and the total score of McGill pain questionnaire were decreased in both groups,and the observation group was lower than the control group(P<0.05);at 3 months after treatment,the score of cervical dysfunction index scale decreased and Barthel index increased in both groups,and the observation group was better than the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Ultrasound-guided selective cervical nerve root block is effective in the treatment of mixed cervical spondylosis,which can effectively reduce pain,improve cervical function and daily living ability,with high safety.
