柴葵清消方治疗非哺乳期乳腺炎的临床疗效及影响因素分析

Analysis of Clinical Efficacy and Influencing Factors of Chaikui Qingxiao Formula for the Treatment of Non-lactating Mastitis

目的:探讨柴葵清消方治疗非哺乳期乳腺炎(NPM)的效果及其影响因素。方法:选取2022年4月—2023年10月西藏自治区人民医院收治的NPM患者140例作为病例组,随机分为试验组与对照组两个亚组,各70例。两组均行切开排脓引流手术,对照组注射地塞米松,试验组口服柴葵清消方。比较试验组与对照组临床疗效、肿块面积、疼痛评分量表(NRS)评分、证候评分;另选取同期140例健康体检者为健康组,对比病例组、健康组临床资料,多因素Logistic回归分析NPM发生的影响因素。结果:试验组治疗总有效率高于对照组(P=0.026);治疗后,两组乳腺肿块面积、NRS评分、证候评分均低于治疗前,且试验组低于对照组(P<0.05)。多因素Logistic回归分析表明,体质量指数(BMI)≥24 kg/m2、初潮年龄≥14岁、乳头内陷、生育次数>1次为NPM独立危险因素。结论:NPM的发病受BMI、初潮年龄、生育次数、乳头内陷等因素影响,柴葵清消方对NPM患者具有良好疗效,能够减轻疼痛并改善临床症状,缩小肿块面积。

Objective:To investigate the effect of Chaikui Qingxiao formula in the treatment of non-lactating mastitis(NPM)and its influencing factors.Methods:One hundred and forty NPM patients admitted to Tibet Autonomous Region People's Hospital from April 2022 to October 2023 were selected as a case group,and were randomly divided into two subgroups of experimental group and control group,each with seventy cases.Both groups underwent incision and drainage surgery,the control group was injected with dexamethasone,and the experimental group was orally administered with Chaikui Qingxiao formula.The clinical efficacy,mass area,pain rating scale(NRS)score,and evidence score of the test group were compared between the experimental group and the control group.In addition,one hundred and forty healthy medical check-ups were selected as the healthy group during the same period of time to compare the clinical data of the case group and the healthy group,and the influencing factors of the occurrence of NPM were analyzed by multifactorial logistic regression.Results:The total effective rate of treatment in the experimental group was higher than that in the control group(P=0.026);after treatment,the area of breast lump,NRS score,and evidence score of both groups were lower than that before treatment,and the experimental group was lower than that of the control group(P<0.05).Multifactorial Logistic regression analysis showed that body mass index(BMI)≥24 kg/m2,age at menarche≥14 years,nipple inversion,and number of births>1 were independent risk factors for NPM.Conclusion:The development of NPM is affected by BMI,age at menarche,number of births,nipple inversion,etc.Chaikui Qingxiao formula has good efficacy in patients with NPM,which can reduce the pain and improve the clinical symptoms,and reduce the size of the lump.
