摘要
目的:探究重症护理超声在预防动-静脉体外膜肺氧合(VA-ECMO)患者下肢静脉血栓的效果。方法:选取2020年4月1日至2022年6月31日符合条件的80例VA-ECMO患者,按照抽签法分为对照组和试验组各40例。对照组采用常规护理干预,试验组在对照组基础上采用重症护理超声技术引导下的针对性护理干预。评估两组患者下肢静脉血栓发生率、下肢静脉血流速度、下肢肌力及血管直径。结果:观察组下肢静脉血栓发生率低于对照组(P<0.05);髂腰肌、臀大肌、内收肌、臀中肌、股四头肌、股二头肌、胫前肌、腓肠肌及比目鱼肌的肌力均高于对照组(P<0.05)。观察组下肢静脉血流速度大于对照组,而血管直径小于对照组(P<0.05)。结论:重症护理超声在预防VA-ECMO患者下肢静脉血栓中的效果较好,可促进患者下肢肌力回复,缩小血管直径,加快下肢静脉血流速度。
Objective:To explore the effect of intensive care ultrasound in the prevention of lower limb venous thrombosis in patients with arteriovenous extracorporeal membrane oxygenation(VA-ECMO).Methods:Eighty eligible VAECMO patients from April 1,2020 to June 31,2022 were selected and divided into a control group and an experimental group with 40 each by drawing lots.The control group received routine nursing intervention,while the experimental group received targeted nursing intervention guided by intensive care ultrasound technology on the basis of the control group.The incidence,blood flow velocity,muscle strength and blood vessel of lower limb vein thrombosis were evaluated in the two groups.Results:The incidence of lower limb venous thrombosis in the observation group was lower than that in the control group(P<0.05);the muscle strength of iliopsoas,gluteus maximus,adductor,gluteus medius,quadriceps femoris muscle,biceps femoris muscle,tibialis anterior,gastrocnemius muscle and soleus in the observation group were higher than those in the control group(P<0.05);the blood flow velocity of lower limb veins in the observation group was higher than that in the control group,while the diameter of blood vessels was smaller than that in the control group(P<0.05).Conclusion:Intensive care ultrasound has a good effect in the prevention of lower limb vein thrombosis in VA-ECMO patients,which can promote the recovery of lower limb muscle strength,reduce the diameter of blood vessels,and accelerate the blood flow speed of lower limb veins.
