黄芪桂枝五物汤结合湿敷法治疗气虚血瘀型淤积性皮炎的临床疗效观察

Clinical effect observation of Huangqi Guizhi Wuwu decoction combined with compound Huangbai lotion in the treatment of stasis dermatitis caused by qi deficiency and blood stasis

目的:探讨黄芪桂枝五物汤结合复方黄柏洗液涂剂湿敷治疗气虚血瘀型淤积性皮炎的临床疗效观察。方法:收集2022年9月至2023年9月于上海市嘉定区外冈镇社区卫生服务中心门诊就诊的气虚血瘀型淤积性皮炎患者60例,随机分为治疗组和对照组各30例。治疗组予以内服黄芪桂枝五物汤结合复方黄柏洗液涂剂湿敷联合治疗,对照组予以常规西药对症治疗,两组均治疗4周。采用湿疹面积严重程度(EASI)评分、瘙痒症状(VAS)评分、中医症候评分评价两组治疗前后症状,并观察临床疗效。结果:治疗后,两组EASI评分、VAS评分、中医症候评分均较治疗前降低(P<0.05),且治疗组明显优于对照组(P<0.05)。治疗组总有效率为100.00%(30/30),高于对照组的76.67%(23/30,P<0.05)。结论:黄芪桂枝五物汤结合复方黄柏洗液涂剂湿敷治疗气虚血瘀型淤积性皮炎,能明显改善患者临床症状,临床疗效良好,具有临床推广价值。

Objective:To observe the clinical effect of Huangqi Guizhi Wuwu decoction combined with compound Huangbai lotion in the treatment of stasis dermatitis caused by qi deficiency and blood stasis.Methods:From September 2022 to September 2023,60 patients with stasis dermatitis caused by qi deficiency and blood stasis were collected in Department of Traditional Chinese Medicine of Waigang Community Health Service Center of Jiading District,Shanghai and randomly divided into a treatment group and a control group with 30 cases in each group.The treatment group was treated with Huangqi Guizhi Wuwu decoction combined with wet compress of compound Huangbai lotion,while the control group was treated with conventional western medicine for symptomatic treatment.Both groups were treated for 4 weeks.Severity of eczema area(EASI)score,pruritus symptom(VAS)score and TCM symptom score were used to evaluate the symptoms of the two groups before and after treatment,and the clinical effect was observed.Results:After treatment,the scores of EASI,VAS and TCM syndrome in both groups were lower than those before treatment(P<0.05),and the treatment group was significantly better than the control group(P<0.05).The total effective rate in the treatment group was 100.00%(30/30),which was higher than that in the control group 76.67%(23/30,P<0.05).Conclusion:Huangqi Guizhi Wuwu decoction combined with compound Huangbai lotion can obviously improve the clinical symptoms of patients with stasis type of stasis dermatitis caused by qi deficiency and blood stasis,the clinical effect is good,which is worthy of clinical promotion.
