基于数据挖掘探讨潘立群辨治胃癌的用药规律

Medication rule of Pan Liqun in syndrome differentiation-based treatment of gastric cancer:A study based on data mining

目的:应用中医传承辅助平台(V3.0)分析潘立群教授治疗胃癌的用药规律。方法:收集并整理2022年1月1日至2022年12月31日潘教授门诊治疗胃癌的医案,运用中医传承辅助平台(V3.0)的关联规则和熵聚类等方法,对数据库中的用药及组方进行统计分析并挖掘核心聚类方药组合。结果:共纳入处方163首,涉及中药216味;中药使用频次排名前10位的药物为甘草、茯苓、白术、陈皮、白芍、石斛、枳壳、桂枝、南沙参、厚朴。中药四气以温、平性为主,五味以甘、苦、辛为主,主要归脾、胃、肺经,功效以补虚、理气、利水渗湿、化痰止咳平喘为主。得出核心药对10个,通过层次聚类分析获得新处方3个。结论:潘教授诊治胃癌以“运脾和胃”贯穿始终,把握整体属虚、局部属实的病机特点,治则以扶正祛邪为主。

Objective:To investigate the medication rule of Professor Pan Liqun in the treatment of gastric cancer using Traditional Chinese Medicine Inheritance Support System(V3.0).Methods:Medical records were collected from the patients with gastric cancer who were treated at the outpatient service of Professor Pan from January 1 to December 31,2022.The methods such as association rule analysis and entropy clustering in Traditional Chinese Medicine Inheritance Support System(V3.0)were used to perform a statistical analysis of the medications and prescriptions in the database and identify core prescriptions and drug combinations.Results:A total of 163 prescriptions were included,involving 216 traditional Chinese medicine(TCM)drugs,and the top 10 TCM drugs in terms of frequency of use were Radix Glycyrrhizae,Poria cocos,Atractylodes macrocephala Koidz.,tangerine peel,Radix Paeoniae Alba,Dendrobium,Fructus Aurantii,Ramulus Cinnamomi,Radix Adenophorae,and Magnolia officinalis.Most drugs had a warm or neutral nature and a sweet,bitter or pungent taste,and they mainly entered the spleen,stomach,and lung meridians,with the main functions of tonifying deficiency,regulating Qi,promoting diuresis and draining dampness,and resolving phlegm and relieving cough and asthma.A total of 10 core drug combinations were obtained,and the hierarchical cluster analysis obtained 3 new prescriptions.Conclusion:Professor Pan consistently uses the method of“activating the spleen and harmonizing the stomach”in the diagnosis and treatment of gastric cancer and masters the pathogenic features of overall deficiency and local excess,with the main principle of strengthening vital Qi and eliminating pathogen.
