摘要
目的探讨瑞芬太尼复合羟考酮用于颅内动脉瘤栓塞术的麻醉效果。方法选取汉中市勉县医院2020年6月至2022年6月收治因蛛网膜下腔出血行颅内动脉瘤栓塞术患者110例,随机分为对照组和观察组,各55例。对照组患者予注射用盐酸瑞芬太尼复合丙泊酚乳状注射液麻醉维持,观察组患者予注射用盐酸瑞芬太尼复合盐酸羟考酮注射液麻醉维持。结果观察组患者麻醉诱导前、手术开始30 min时、手术结束时及拔管后5 min的平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、脉搏氧饱和度(SpO_(2))、脑电双频指数(BIS)均显著优于对照组(P<0.05);术中血管活性药物使用率显著低于对照组(P<0.05);术后呼唤苏醒时间、拔管时间、定向力恢复时间均显著短于对照组(P<0.05);不良反应发生率显著低于对照组(P<0.05)。结论瑞芬太尼复合羟考酮用于颅内动脉瘤栓塞术麻醉,能维持患者术中血流动力学稳定,减少血管活性药物的使用,促进术后恢复。
Objective To investigate the anesthetic effect of remifentanil combined with oxycodone in intracranial aneurysm embolization.Methods A total of 110 patients underwent intracranial aneurysm embolization due to subarachnoid hemorrhage admitted to the Mian County Hospital from June 2020 to June 2022 were selected and randomly divided into the control group and the observation group,with 55 cases in each group.The patients in the control group were given Remifentanil Hydrochloride for Injection combined with Propofol Injectable Emulsion for maintenance of anesthesia,while the patients in the observation group were given Remifentanil Hydrochloride for Injection combined with Oxycodone Hydrochlorice Injection for maintenance of anesthesia.Results The mean arterial pressure(MAP),heart rate(HR),respiratory rate(RR),pulse oxygen saturation(SpO_(2))and bispectral index(BIS)in the observation group were significantly better than those in the control group before anesthesia induction,30 min after the start of surgery,at the end of surgery,and 5 min after extubation(P<0.05);the utilization rate of vasoactive drugs during surgery was significantly lower than that in the control group(P<0.05);the postoperative wake-up time,extubation time,and orientation recovery time was significantly shorter than that in the control group(P<0.05);the incidence of adverse reactions was significantly lower than that in the control group(P<0.05).Conclusion The application of remifentanil combined with oxycodone in intracranial aneurysm embolization can maintain the hemodynamic stability during surgery,decrease the use of vasoactive drugs,and promote the postoperative recovery.
