2010—2021年辽宁省庄河市胃癌筛查人群胃黏膜病变病理变化谱分析

Pathological Changes in Gastric Mucosal Lesions Detected Among Gastric Cancer Screening Population in Zhuanghe City of Liaoning from 2010 to 2021

[目的]分析辽宁省庄河市2010—2021年胃癌筛查人群中胃黏膜病变病理变化谱。[方法]纳入2010年1月至2021年12月期间辽宁省庄河市胃癌筛查受检者,所有受检者均按照筛查方案接受胃镜检查和/或活检取材。计算不同病变构成比,分析胃癌筛查人群胃黏膜病变病理谱特征,应用Joinpoint回归模型分析其时间变化趋势。[结果] 2010—2021年辽宁省庄河市胃癌筛查受检者共20 986人,女性(12 569人)多于男性(8 417人),且50岁及以上者居多。非萎缩性胃黏膜病变女性多见,上皮内瘤变及胃癌中男性占比更高。2010—2021年庄河市非萎缩性胃黏膜病变占比上升(AAPC=6.85%,95%CI:1.61%~12.36%),低级别上皮内瘤变占比下降(AAPC=-11.65%,95%CI:-17.27%~-5.66%),趋势变化均有统计学意义(P均<0.05)。[结论] 2010—2021年辽宁省庄河市胃黏膜病变病理变化谱随年代变化呈一定的变化趋势,其中非萎缩性胃黏膜病变占比呈上升趋势,而低级别上皮内瘤变呈下降趋势。

[Purpose]To analyze the pathological changes of gastric mucosal lesions detected in individuals undergoing gastric cancer screening in Zhuanghe City of Liaoning Province from 2010 to 2021.[Methods]Participants undergoing gastric cancer screening in Zhuanghe from January 2010 to December 2021 were included in the study.All participants underwent endoscopic examination and/or biopsy according to the screening protocol.The proportion of different lesions was calculated to analyze the pathological spectrum characteristics of gastric mucosal lesions in the gastric cancer screening population in Zhuanghe.The trend was analyzed using the Joinpoint regression model.[Results]A total of 20968 participants were included,with a higher proportion of females than males,and the majority being aged 50 years or older.Non-atrophic gastric mucosal lesions were more prevalent among females.From 2010 to 2021,the proportion of non-atrophic gastric mucosal lesions in Zhuanghe significantly increased(AAPC=6.85%,95%CI:1.61%~12.36%),and the proportion of low-grade intraepithelial neoplasia significantly decreased(AAPC=-11.65%,95%CI:-17.27%~-5.66%)(both P<0.05).[Conclusion]From 2010 to 2021,the proportion of non-atrophic gastric mucosal lesions detected in screening population of Zhuanghe City shows an increasing trend,while that of low-grade intraepithelial neoplasia shows a downward trend.
