老年社区获得性肺炎患者血清可溶性髓系细胞表达触发受体1、正五聚蛋白3、降钙素原/血清白蛋白比值与病情严重程度的关系分析

Relationship between serum sTREM-1,PTX3,PCT/Alb ratio and disease severity in elderly patients with community-acquired pneumonia

目的探讨老年社区获得性肺炎(CAP)患者可溶性髓系细胞表达触发受体1(sTREM-1)、正五聚蛋白3(PTX3)、降钙素原/血清白蛋白(PCT/Alb)比值与病情严重程度的关系。方法选取2021年4月至2024年4月在新疆维吾尔自治区人民医院治疗的老年CAP患者108例为观察组,根据英国胸科协会改良肺炎评分(CURB-65)将患者分为轻症组(72例)与重症组(36例)。另选择100例同时期在相同医院体检的健康者为对照组。比较各组间人群的sTREM-1、PTX3以及PCT/Alb比值水平,采用Pearson相关性分析sTREM-1、PTX3以及PCT/Alb比值与病情严重程度的相关性,绘制受试者工作特征(ROC)曲线分析sTREM-1、PTX3以及PCT/Alb比值对重度感染的诊断价值。结果观察组血清sTREM-1、PTX3以及PCT/Alb比值均高于对照组(均P<0.05);重症组患者血清sTREM-1、PTX3以及PCT/Alb比值较轻症组显著升高(均P<0.05);Pearson相关性分析显示,血清sTREM-1、PTX3以及PCT/Alb比值与病情严重程度均呈正相关(r=0.406、0.546、0.479,均P<0.05);3项血清指标联合诊断重症CAP的ROC曲线下面积(AUC)为0.933。结论老年CAP患者sTREM-1、PTX3以及PCT/Alb比值均明显升高,与病情呈正相关;3项指标联合诊断重症CAP的准确性更高。

Objective To explore the relationship between soluble triggering receptor expressed on myeloid cell-1(sTREM-1),pentraxin-3(PTX3),procalcitonin/serum albumin(PCT/Alb)ratio and disease severity in elderly patients with community-acquired pneumonia(CAP).Methods A total of 108 elderly patients with CAP in People′s Hospital of Xinjiang Uygur Autonomous Region were enrolled as observation group between April 2021 and April 2024.According to scores of confusion,uremia,respiratory,BP,age 65 years(CURB-65),they were divided into mild group(72 cases)and severe group(36 cases).A total of 100 healthy controls during the same period and hospital were enrolled as control group.The levels of sTREM-1,PTX3 and PCT/Alb ratio were compared in different groups and among patients with different disease severity.The correlation between sTREM-1,PTX3,PCT/Alb ratio and disease severity was analyzed by Pearson correlation analysis.The diagnostic value of sTREM-1,PTX3 and PCT/Alb ratio for severe infection was analyzed by receiver operating characteristic(ROC)curves.Results The levels of serum sTREM-1,PTX3 and PCT/Alb ratio in observation group were higher than those in control group(P<0.05),and which were significantly increased in severe group when compared to mild group(P<0.05).Pearson correlation analysis showed that levels of serum sTREM-1,PTX3 and PCT/Alb ratio were positively correlated with disease severity(r=0.406,0.546,0.479,P<0.05).The area under the curve(AUC)of sTREM-1 combined with PTX3 and PCT/Alb ratio in the diagnosis of severe CAP was 0.933.Conclusions The levels of sTREM-1,PTX3 and PCT/Alb ratio are significantly increased in elderly patients with CAP,which are positively correlated with disease severity.Combined detection of the three indexes has higher accuracy for severe CAP.
