康复新液联合曲安奈德辅助硫酸羟氯喹治疗糜烂型口腔扁平苔癣的疗效

The therapeutic effect of Kangfuxin liquid combined with triamcinolone acetonide and hydroxychloroquine sulfate in the treatment of erosive oral lichen planus

目的探讨康复新液联合曲安奈德辅助硫酸羟氯喹治疗糜烂型口腔扁平苔癣的疗效。方法选择浙江省绍兴市柯桥区中医医院医共体总院2021年12月~2023年12月收治的糜烂型口腔扁平苔癣患者88例,随机分为羟氯喹组和联合治疗组,每组44例。羟氯喹组采用硫酸羟氯喹单一口服治疗,2次/d,200mg/次;联合治疗组在羟氯喹组基础上将康复新液与曲安奈德按1:1混合,每次取10mL含漱5分钟,3次/d。两组均治疗4周,比较治疗效果。结果(1)治疗前,两组口腔糜烂面积和疼痛评分差异无统计学意义(P>0.05);治疗2周及4周,联合治疗组口腔糜烂面积小于羟氯喹组(P<0.01),疼痛评分低于羟氯喹组(P<0.01);(2)治疗前,两组IFN-γ、IgA、IgM、IgG免疫调节因子差异无统计学意义(P>0.05),治疗4周后联合治疗组IFN-γ、IgA、IgM、IgG高于羟氯喹组(P<0.05或P<0.01);(3)治疗前,两组TNF-α、IL-10、IL-17差异无统计学意义(P>0.05),治疗4周后,联合治疗组以上炎性因子水平均低于羟氯喹组(P<0.01)。结论康复新液联合曲安奈德辅助硫酸羟氯喹治疗糜烂型口腔扁平苔癣具有较好的疗效,可通过调节免疫功能,减轻口腔黏膜的炎症反应达到治疗目的。

Objective To investigate the therapeutic effect of the combination of hydroxychloroquine sulfate,rehabilitation solution,and triamcinolone acetonide in the treatment of erosive oral lichen planus.Method 88 patients with erosive oral lichen planus admitted to the Medical Community General Hospital of Keqiao District Traditional Chinese Medicine Hospital in Shaoxing City,Zhejiang Province from December 2021 to December 2023 were randomly divided into a hydroxychloroquine group and a combination therapy group,with 44 patients in each group.The hydroxychloroquine group received a single oral treatment regimen of hydroxychloroquine sulfate,twice a day,200mg/time.The combination treatment group received a 1:1 mixture of rehabilitation solution and triamcinolone acetonide based on the hydroxychloroquine group.10mL of the mixture was taken and rinsed for 5 minutes each time,three times a day.Both groups lasted for 4 weeks and the treatment effects were compared.Result(1)Before treatment,there was no statistically significant difference in erosion area and pain score between the two groups(P>0.05),After 2 and 4 weeks of treatment,the erosion area in the combination therapy group was smaller than that in the hydroxychloroquine group(P<0.01),and the pain score was lower than that in the hydroxychloroquine group(P<0.01);(2)Before treatment,there was no statistically significant difference in the immune regulatory factors of IFN-γ,IgA,IgM,and IgG between the two groups(P>0.05),After 4 weeks of treatment,the combination therapy group had higher levels of IFN-γ,IgA,IgM,and IgG than the hydroxychloroquine group(P<0.05 or P<0.01);(3)Before treatment,there was no statistically significant difference in the inflammatory factors between the two groups(P>0.05),After 4 weeks of treatment,the levels of TNF-α、IL-10 and IL-17 in the combination therapy group were lower than those in the hydroxychloroquine group(P<0.01).Conclusion The combination of Fuxin solution and triamcinolone acetonide with hydroxychloroquine sulfate has a good therapeutic effect on erosive oral lichen planus,which can be achieved by regulating immune function and reducing the inflammatory response of oral mucosa.
